We have a new position for a Care Coordinator Registered Nurse with Main Line HealthCare's Practice Performance and Quality Department

This position is based out of the Corporate office in Newtown Square, PA with travel to Practice locations required as needed and start time of 9:00am or 9:30am

The Registered Nurse will be working Full Time, 40 hour/week position Monday-Friday, Day shift. No holiday requirements

The Care Coordinator RN participates as the ambulatory care coordinator of the outpatient team of professionals to meet health goals of patient and families, monitors outcomes and utilization of services, and maintains communication with providers of care, practice staff and payers. The care coordinator utilizes age appropriate clinical assessment and intervention with patients to support their psycho-social and medical needs of identified patients and families.

Responsibilities to include but are not limited to:

Program Development:

  • Supports the executive director in designing, implementing, and monitoring MLHC's care management services and activities for high risk patient populations; focusing on patients with chronic diseases and high medical service utilization.

Care Management Activities

  • Promotes "seamless" patient care through enhanced provider continuity, effective and timely communication with the care team, and routine planned care visits.

  • Conducts initial and periodic telephonic outreach to eligible patients focusing on clinical assessments, early symptom recognition and intervention, self management abilities, and health literacy.

  • Communicates problems or concerns with patient's care team/ specialists, ancillary services as appropriate.

  • Collaborates with the provider and patient to establish, implement, and sustain a customized plan of care using evidence-based guidelines.

  • Initiates appropriate evaluation and management of urgent/ emergency situations.

  • Educates patients/families on current medications and treatment plans, including health and illness prevention.

  • Utilizes clinical skills to determine patient and families psychosocial and financial status, support systems

  • Refers patients and families to appropriate community resources based on identified and anticipated needs.

  • Serves as a patient/family advocate in the health care and community resource system.

Education:

  • Registered Nurse License Required, Bachelor's Degree Preferred

Experience:

  • Minimum of two years recent experience in telephonic care management outreach required;

  • Minimum of two or more years of recent home care experience preferred.

  • Minimum of two years recent insurance, case management experience preferred.

Title: Registration Specialist The Registration Specialist is the initial contact for patients entering the Main Line Health System through various portals within the organization. This position acts as ambassadors for patient education as it relates to financial responsibility, customer service, service recovery and follow through for patient satisfaction. This role is responsible for accurate data input ensuring all financial and regulatory requirements are met. Essential Accountabilities: * Interviews patients and or guarantors to gather complete and accurate financial and demographic information. * Contacts other resources when necessary such as physician's office and or other medical facilities to obtain the required admission/registration information. * Ensures third party pre-certification, insurance verification and other billing requirements are met. * Responds to inquiries from patients, employees and the general public regarding directions/locations, foreign language interpreters, advance directives, and services available at the facility. * Launches reviews, accepts or rejects HDX insurance information. * Ensures ED level of care is entered, seeks assistance from nursing when required. * Uses Smart Chart's Service Provider to verify admit order, admit type and enter in system accordingly when required. * Provides and promotes professional, efficient and congenial customer service through all communication and interaction with patients, patient's family physicians, visitors, and co-workers. QUALIFICATIONS: Education: High School diploma preferred. Experience: * Minimum of two year's registration experience in a medical setting is preferred. * Knowledge of personal computer and relevant software applications is required. * Two years working in customer service is required. * Knowledge of medical terminology preferred. * Knowledge of insurance and third party reimbursement preferred. * Excellent customer service skills and computer skills required.
