JOB SUMMARY: Utilizes internal and external resources to assist older adults to function at an optimum level of independence through comprehensive assessment, care planning, linkage with essential services and monitoring of service delivery outcomes. The incumbent will be a resource regarding services including but not limited to emergency relief, housing, eligibility/securing entitlements and caregiver supports.
Knows and supports the Catholic Charities of Buffalo NY mission, vision and guiding principles;
Demonstrates ability to increase skills related to interventions necessary to provide effective service;
Maintains active caseload of assigned clients and ensures all services are in compliance with contract requirements, and accreditation standards;
Conducts comprehensive assessments including reviewing referral information, identifying and engaging key participants, identifying systemic strengths and needs and the external factors impacting the consumer's goal attainment;
Utilizes person-centered practices to assist the enrollee with the development of an Individualized Services Plan (ISP) addressing:
Services, supports and providers that will help in recovery, wellness and satisfaction;
Activities, services, and supports based upon the individual's interests, preferences, and strengths;
Inclusion of those who are important to the individual in lifestyle planning;
Opportunities to exercise control and make informed decisions from meaningful choices; and
Skills needed to achieve personal relationships, community inclusion, dignity, and respect;
Schedules and facilitates service plan reviews, evaluates the advances and barriers to intervention effectiveness and revises the plan as appropriate;
Maintains clear and concise documentation of interventions that demonstrates compliance with the principles of interdisciplinary team as well as funder requirements and accreditation standards;
Acts as an advocate for clients in dispute resolution;
Utilizes data to identify areas of needed improvement and implements improvement plans in support of achieving performance targets, program goals, client satisfaction and positive client outcomes;
Provides community education including but not limited to Powerful Tools for Caregivers and other evidence based curricula, to enhance public knowledge, caregiver support, outreach and public relations;
Complies with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your job duties.
Establishes and maintains cooperative and collaborative relationships with co workers/professionals within Catholic Charities and in peer organizations and professional associations;
Maintains currency in required fields of professional expertise and competencies including but not limited to: Medicaid, Medicare, federal, state and local regulations, and community resources.
ACCOUNTABILITIES/SUPERVISORY RESPONSIBILITIES: Works under the direct supervision of the Program Supervisor and in cooperation with the interdisciplinary team.
EDUCATION: Bachelor' degree in social work, mental health counseling, gerontology or other approved related field preferred . Persons with a related Associate's degree and significant experience in the targeted areas may also qualify at the discretion of the agency.
EXPERIENCE: Three (3) years direct experience linking clients age 60+ to a broad range of essential, community based services.
Basic understanding of psychiatric and substance use disorders in older adults;
Willingness to be part of a team approach;
Demonstrated compassionate behavior toward elderly persons in need;
Flexibility and willingness to learn;
Excellent interpersonal and team building skills.
Willingness to provide services in the community throughout the City of Buffalo and suburbs;
NYS driver license and maintenance of reliable personal transportation.
