Responsibilities

The Care Coordinator is responsible for coordination of care across the continuum during the patient's acute, chronic and long-term stages of illness for a defined patient population. This includes utilization

management, transitional care planning, psychosocial and functional status assessment, patient advocacy, education for the patient/family, and monitoring quality indicators to demonstrate outcomes resulting from the service provided. Collaborates with all team members to provide a comprehensive assessment of the patient's plan of care, goal/outcome attainment and continued care needs.

Qualifications

Required Minimum Education: RN with a Georgia License, Graduate of an accredited/approved school of nursing:

Baccalaureate degree in nursing (BSN) from an accredited school of nursing preferred, or MSW from an Accredited School of Social Work Licensure: RN with a Georgia License or LMSW,

Required Minimum Experience: Three to Five years Clinical Practice/Experience is preferred.

Required Minimum Skills:
Knowledge of Case Management process.

Excellent organizational and professional communication skills. Knowledgeable in utilizing screening criteria in review of clinical data with respect to patients/clients needs for health care. Ability to effect change performs critical analysis, promote client/family autonomy and plan and organize effectively for the continuum of care.

Responsibilities The Care Coordinator RN Sr. will manage a case load and workflow centered around the sickest and most complex cases requiring strong clinical expertise to partner with the physician and care team members to drive the most optimal and efficient treatment plan to streamline safe and timely throughput while in the hospital as well as planning for post discharge care. Will assume more of a lead care coordinator role during complex care conference and will lead departmental and hospital-based QI initiative that better optimizes patient throughput and outcomes prevent avoidable delays that have potential to cause patients harm. This staff member will serve as an expert resource for complex patients/situations and as a guide/consultant to the other care team members surrounding patient psychosocial and resource needs. Is an expert with supporting community-based service offerings (indigent care, support groups, specialty care) and offers guidance to patients/families as well as other care team members. Is responsible for selected functions related to Advance Directives. The Care Coordinator RN Sr, is responsible for providing psychosocial assessment for patients with the appropriate planning for any required follow up care. This staff member is also responsible for collaborating with healthcare providers and members to promote quality patient outcomes, to optimize patient plan benefits by enrolling in any payer post discharge programs and to promote effective use of resources. Primary duties may include, but are not limited to: Conducts pre-certification, continued stay review, care coordination, or discharge planning for appropriateness of treatment setting reviews to ensure compliance with applicable criteria, medical treatment plan. Ensures patient has access to medically necessary and appropriate care, quality healthcare in a cost-effective setting, right care, right place and right time. Consult with other clinical experts, physicians and physician advisors to ensure medically appropriate, high quality, cost effective care throughout the medical management process. Collaborates with providers to assess members needs and early identification of discharge planning needs. Facilitates member care transition through the healthcare continuum. Qualifications Required Minimum Education: A Bachelor of Nursing Degree from an accredited school and RN License in the State of Georgia is required. Required Minimum Certification: Currently licensed as a Registered Nurse in the State of Georgia or hold a privilege to practice in the State of Georgia under the Enhanced Nurse Licensure Compact (eNLC). Required Minimum Experience: At least three years of experience as a staff nurse in an acute care hospital setting. Three years of experience as a case manager in a hospital setting or payer-based model with expertise in case management competencies and to guide the care team through complex discussions. Required Minimum Skills: Excellent verbal and written communication skills. Knowledgeable regarding local, state and federal regulations required. Knowledge of community and state-wide resources and programs. Ability to function well under stress. Self-directed. Ability to build and sustain trust with physicians and other care team members.
