Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost effective health outcomes.
Master's degree in behavioral health or health care field.
Minimum two years' clinical experience.
Self-starter with strong verbal and written communication skills.
Comfort with change and quality improvement processes.
Experience with Electronic Health Records strongly preferred.
Experience with Motivational Interviewing strongly preferred.
Knowledge of New Mexico and local community resources strongly preferred.
Bilingual (English / Spanish) or (English / Navajo) preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
o RN license in New Mexico; OR
o Independent license as Mental Health professional in New Mexico (e.g., LCSW, LISW, LPCC, LMFT, or LCP).
This position requires successful completion of an initial post-offer of employment TB Test (and upon exposure in accordance with PMS Policies).
According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
