About the Program

At Hillcrest, we strongly advocate for the wellness of our clients through our Integrated Health Home (IHH) program that delivers the following services, and more, using an individual/family-centered, strengths-based approach. For children and adults, our services include:

  • Treatment of particular mental illnesses including anxiety, depression, bipolar syndrome and ADHD

  • Comprehensive care management and coordination of our client's health including coordination of doctors, therapists and prescribers

  • Health promotion and wellness prevention activities

  • Comprehensive transitional care

  • Individual and family support services

  • Referral to community and social support services

  • A Hillcrest team of nurses, care coordinators and peer group specialists to make certain all wellness needs are being taken care of

Your Responsibilities

As a Care Coordinator, your primary responsibility includes providing mental and physical health care and services for assigned caseload as part of the IHH team. In addition, you will:

  • Serve as a member of the interdisciplinary team, working closely with nursing, primary health care and auxiliary service providers.

  • Assist with the enrollment process, which includes paperwork.

  • Coordinate appointment scheduling, arrange transportation, and make follow-up calls as necessary.

  • Provides continued education and reinforces ongoing whole health needs.

What you need

  • Bachelor's degree in social work, sociology, or psychology

  • Valid driver's license and access to a reliable vehicle

  • Ability to communicate clearly and professionally to clients, staff, families, consulting psychiatrist and other professional staff.

You will have an opportunity to immediatly impact the team's success by delivering your highest quality of services within the maintenance area, demonstrating a willingness to assist where needed. In the process, you will be given the opportunity to learn more skilled areas within the maintenance and repair process, including plumbing, carpentry, electrical and other skilled building trades areas. Your attention to detail, willingness to learn and ability to help where needed will be key to your success. Among many responsibilities you will order and deliver departmental supplies, maintain and repair property, equipment, grounds and entrances, complete maintenance work orders and perform preventative maintenance as assigned. You may be required to with other environmental department tasks which may include housekeeping, laundry and floor care. You may be required to drive company vehicles for transportation, errands and deliveries. Job Requirements At least two years of prior maintenance and facilities or environmental services experience is required with preference for vocational training in construction trades and/or building maintenance. This position requires a valid Iowa Driver's License with a satisfactory driving record. You will need to be able to meet the physical requirements of this position to include being able to travel between floors and areas of the campus, as well as lifting up to 50 pounds without assistance. Because you work in the homes of those we serve, we will need you to be able to read, write and follow written and verbal instructions in English.
