Job Description

JOB SUMMARY:

With specific direction and routine supervision from the Director of Community Health Systems, the CCU Care Coordinator performs Community Care Program case management services which includes professional assessments of the physical, social and economic needs of individual program applicants and/or clients and determines financial eligibility for these services, as well as, the "Choices for Care" client screenings for possible long term placement and the determination of need and eligibility for client financial management services provided a local community based senior services agency. This position is a professional job that requires the employee to exercise appropriate discretion and judgement in the performance of his/her assigned duties and responsibilities.

EDUCATIONAL REQUIREMENTS

Bachelor's Degree in Social Work, Social Science or related field or College Diploma in Nursing plus one year of program experience or Certificate in Practical Nursing plus one year of program experience

CERTIFICATION & LICENSURE REQUIREMENTS

Registered Professional Nurse (RN) License Illinois, if degree is in Nursing upon hire or Licensed Practical Nurse (LPN) Illinois, if degree is in Practical Nursing upon hire and Driver's License upon hire and Proof of Auto Insurance upon hire.

SKILLS AND KNOWLEDGE

Requires working knowledge of professional social service theory and practices.Requires the ability to plan and organize work effectively.Requires a working knowledge of the IDOA guidelines.Requires knowledge of the community service network.Requires good written and oral communication skills.Requires the ability to be flexible in meeting work demands.Requires the ability to comprehend program planning, develop program methods and procedures, and establish appropriate service and referral priorities.Requires the ability to communicate well and establish effective working relationships with staff and supervisors, as well as the general public.Requires the ability to effectively facilitate job productivity.Requires the ability to keep records and charts and make reports and observations.Requires the ability to apply general nursing techniques and practices.Requires the ability to perform computer functions required for the Community Care Program. Requires the ability to respect and adapt readily to a diverse variety of complicated patient care situations. Requires the employee to recognize and respect the confidentiality of all client or patient records, as well as the confidentiality and/or privacy of co-worker's records.Requires the delivery of all services in a tolerant and consistent manner.

ESSENTIAL FUNCTIONS:

  • Provides case management services as designated in the Illinois Department on Aging Case Management Manual for person's age 60 or older.

  • Provides the appropriate on-site/in-home assessments (from eight categories) to determine the needs of the client and the eligibility status.

  • Develops a care plan for each client and arranges in-home services, adult day care services and other services needed to maintain client support.

  • Makes appropriate referrals to other agencies when necessary.

  • Provides program information to the client/family and acts as an advocate for the client.

  • Monitors clients and provides follow-up services in a timely manner.

  • Reports incidents of elder abuse to the designated Adult Protective Services Agency.

  • Maintains program records as outlined by IDOA, ECIAAA and the VCHD.

  • Completes appropriate paperwork in a timely manner.

  • Assists in the chart audits and monthly monitoring.

  • Attends and participates in required training set forth by CRIS, the IDOA and ECIAAA.

  • Maintains the appropriate certification to provide program services.

  • Collaborates with CCU Logistics Coordinator/Case Aide.

  • Collaborates with all other CRIS specific programs, ensuring that CCU participants who qualify are referred to the programs that best fit their needs.

  • Performs other duties as assigned by the Director of Community Health Systems.

Education requireduirements:

  • be an RN, or a BSN, or have a BA/BS degree in social science, social work, or related field. One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or
  • be a LPN with one year of program experience which is defined as assessment of and provision of formal services for the elderly and/or authorizing service provision; or
