GENERAL SUMMARY:

This position will service physician practices located in the Beaumont Dearborn region.

Coordinates the care for groups of patients as it relates to disease management, resource utilization, access, discharge planning, quality, service and efficiently across the continuum. This is an interventional role maintaining a patient centered focus.

ESSENTIAL DUTIES:
1.In collaboration with patient, family, physician, and other health care team members formulates and facilitates the discharge planning activities of assigned patients.
2.Coordinates care and monitors the utilization of resources for patients, physicians, and third-party payers.
3.Utilizing the nursing process and critical thinking skills, coordinates the care for assigned patients.
4.Utilizes established criteria to identify and facilitate the appropriated level of care for assigned patients. Educates patient, family, and health care team members regarding appropriate level of care.
5.Collects and aggregates data via Allscripts automated case management system and electronic medical record documentation.
6.Serves as an educator on the Care Management process to patients/families and other health care team members.
7.Identifies problematic discharges where there may be issues or obstacles which impact or prolong LOS. Attends unit based problematic discharge meeting. Identifies potential risk and intervenes with system inefficiencies. Collects data as indicated to support recommended changes.

A. Education / Training: Bachelor of Science in Nursing.

B. Work Experience: None required.

C. Certification, Licensure, Registration: Current and unrestricted State of Michigan Registered Nurse license, Current American Heart Association BLS for Healthcare Providers certification.

D. Other Qualifications:

The Care Coordinator (RN/SW) must have relevant experience and education to work with Enrollees with complex health, behavioral health, long-term services and supports and/or psychosocial needs and perform the following functions: * Provides access to a single point of contact for all questions or inquiries. * Conducts assessments with Enrollees and/ or their care giver. * Develops an Individualized Care Plan that is periodically reviewed and updated. * Provides disease self-management and coaching. * Conducts medication review, including reconciliation during transitions of care setting. * Providess periodic monitoring of health, functional and mental status along with pain and fall screening. * Ensures the provision of services in the least restrictive setting and transition support across and between specialties and care settings. * Connects Enrollees to services that promote community living and help to delay or avoid nursing facility placement. * Coordinates with social service agencies (e.g., local departments of health, social services and community based organizations) and the referral of Enrollees to state, local and/or other community resources; and * Collaborates with nursing facilities to promote adoption of evidence-based interventions to reduce avoidable hospitalization, management of chronic conditions, medication optimization, fall and pressure ulcer prevention, and the coordination of services beyond the scope of the nursing facility benefit. Education/Experience: * Bachelor's Degree. * Registered Nurse. * A bachelors (or higher) degree in a health related field and licensure as a health professional (where such licensure is available); or * Certification as a case manager (as documented and accepted on URAC's website@ www.urac.org); or * MSW licensure and three (3) years professional practice experience; and * Active state RN license. * Valid driver's license with car insurance. * Current unrestricted Social Worker license. * 1-3 years Social services and/or clinical experience working with complex populations, including those with physical health, behavioral health, long-term services and supports and/or psychosocial needs. * 3-5 years of Case Management preferred.
