Summary

At the direction of professional Care Management and Social Work staff, conducts and documents socio-economic review and screening to support the coordination of care

This position is PRN for the Discharge Lounge

Job Duties

  • Coordinates with home care agencies, post-acute care facilities, durable medical equipment companies, transportation agencies and others as indicated

  • Assists clients and patients in connecting with appropriate Physicians, insurers, Payer programs, and other resources to meet social and clinical needs

  • Collaborates with the care management team for effective patient or client outreach, care coordination and hand-overs across the continuum of care

  • Refers patients for smoking cessation counseling and provides educational resources to patients per protocols

Requirements

  • High School diploma or GED or equivalent

  • None required

  • Preferred: 1 year relevant experience

Provides superior and proactive customer service to our customers, partners and internal teams. Performs all necessary logistical functions including planning, securing transportation and problem resolution. CUSTOMER SERVICE: Acts as the primary point of contact for all customer service throughout move process. Required Experience Education: High School Diploma or equivalent required. Bachelor's Degree in International Business, Business Administration or Mathematics preferred. Three years of relevant experience acceptable in lieu of formal education.. Knowledge/Experience: Must have sufficient understanding of business principles. Prior International Forwarding experience is desired Licenses/Certifications/Specialized Training: Proficiency in Microsoft Office (Word, Excel, Outlook).
