Responsibilities

The Care Coordinator Assistant provides clerical, secretarial and administrative support for the Care Coordination department. Under the direction and supervision of the licensed staff, the Care Coordination Assistant performs the following activities and processes ; post-acute care coordination (SHF, HHC, DME, Hospice , etc., ) revenue related activities ( authorizations, certifications and payer communications ) , stocking and monitoring office supplies and equipment and maintaining accurate and complete departmental records.

This position may function as the point of contact for the department, and is responsible for the professional and timely response to calls, messages and the forwarding of communications to the responsible party. The Care Coordinator Assistant may be assigned other duties within the scope of the role, including, but not limited to, scheduling, faxing, auditing, transcribing, filing, delivering, sorting, editing, tracking and correlating the department's documentation, records and forms.

This positions emphasis will be on optimizing the licensed staff's productivity by providing clerical support, managing confidential patient information and ensuring the compliant and accurate management of departmental records and communications.

The Care Coordinator Assistant performs these duties according to applicable hospital policies, standards of practice and Federal and State regulations.

Qualifications

REQUIREMENTS:

  • Two years of vocational training, medical office experience or relevant work experience required.

  • Demonstrated experience in Microsoft Office, typing, and computer data entry.

  • High School Graduate or equivalent education.

  • Associates Degree preferred.

  • Excellent customer service and presentation skills are a must

  • Strong interpersonal and written communication skills are essential

  • Demonstrated ability to apply analytical and problem solving skills

  • Demonstrated ability to manage multiple tasks or projects effectively

  • Ability to work independently as needed with a high degree of detail orientation.

  • Ability to work efficiently in a fast-paced environment with changing priorities

Responsibilities The incumbent demonstrates competency in patient care for neonate, pediatric, adult, and geriatric patients. The incumbent will perform arterial punctures, continuous ventilation, airway maintenance, emergency care, infection control, isolation procedures, maintenance and operation of mechanical ventilators, cardiopulmonary resuscitation, aerosolized medication delivery for the treatment of pulmonary disease, and medical gas administration. The incumbent will access the medications from the medication storage unit and the nurse servers in the patient care areas. Performs other related duties as assigned or requested in professional manner. The incumbent shall demonstrate behaviors consistent with the core (Mercy) values in support with Mercy San Juan Medical Center and the Mission and Philosophy of Dignity Health.QualificationsTo qualify for this position, completion of an AMA approved respiratory therapy program is required. 2 years experience in an acute care hospital setting with 1 year being critical care. If no, qualified candidate a training position maybe considered, depending on staffing needs. Current and in good standing, license with the California boards of Respiratory care, BLS card issued from the AHA. Required: Copy of diploma at highest level of academic achievement related to clinical function of licensee OR transcript showing awarding of degree from the school where the degree (highest level of academic achievement related to clinical function of licensee) was granted is required.
