SCHEDULE: 40 hours, 8am
Pine Street Inn Housing provides comprehensive, intensive case management support to chronically homeless individuals with histories of substance abuse, mental illness, and chronic complex medical conditions in housing using the Housing First model. The Behavioral Health Community Partner Care Coordinator (BHCP Care Coordinator) will work using Motivational Interviewing, Trauma-Informed care and Harm Reduction best practice models to engage members living in PSIs housing portfolio with the purpose of enrolling Mass Health members in a new Initiative that provides qualified individuals with a service benefit called Behavioral Health Community Partners (BH CP). The BHCP Care Coordinator role will be community-based and serve as a vital link connecting homeless and formerly homeless individuals struggling with serious mental illness, substance use disorders, and/or complex medical illnesses to their primary care team, behavioral health providers and other medical specialists. The BHCP Care Coordinator will also work closely with the enrollee to identify services that address social determinants of health needs. The BH CP Care Coordinator will work as part of an interdisciplinary BHCP team that will include the BH CP medical care team at Boston HealthCare for the Homeless. The BHCP Care Coordinator will manage a caseload between 40-50 enrollees and be responsible for working with individuals to coordinate their care. The BHCP Care Coordinator will work closely with nurse care managers, physicians and social workers. The focus of care will include 1) engaging enrollees regularly (several times a month), mostly face to face 2) ensuring that the enrollee remain connected to vital services and 3) ensures the progression towards established goals outlined in their Person Centered Care Plan.
Associates Degree in related field
Strong written communication skills
Computer Proficiency in Microsoft Office Products
A valid Mass drivers license in good standing
3-5 years experience in case management, human services or community based health center setting
Experience working with individuals experiencing substance abuse, mental illness, trauma, homelessness, chronic medical conditions and/or other complex disorders.
Strong commitment to learning and using harm-reduction and motivational interviewing skills in order to promote improved medical and behavioral health among BH CP enrollees.
Ability to set limits fairly and consistently while maintaining appropriate boundaries.
Knowledge of effective crisis intervention techniques and ability to respond appropriately in crisis situations.
Excellent organizational, communication, and interpersonal skills; the ability to balance many competing demands.
Self-directed with the ability to work both independently and as part of a team
Ability to access different building locations and program sites via vehicle, public transit, and by foot in varied weather conditions, including climbing several flights of stairs and lifting up to 50 pounds of weight
Ability to sit for long periods of time and use computer, calculator, fax, copier and other office equipment
Ability to communicate professionally both verbally and in writing, including the ability to successfully represent organization in various forums
Ability to juggle many competing demands, to prioritize work efficiently and effectively
Ability to take initiative, plan and work independently
