WellStar is looking to add a Care Coordination Assistant to its West Georgia Medical Center team!!
The Care Coordination Assistant is responsible for selected functions related to Advance Directives, Utilization Review Education and Disposition Planning. The Care Coordination Assistant communicates and collaborates with Care Coordinators and other disciplinary team members.
The Care Coordination Assistant's responsibility is to support the Care Coordinators and patients requesting assistance with advance directives. Support duties may include assisting with utilization management, transition of care, pharmaceutical applications and other duties as assigned by Supervisor. The Care Coordination Assistant may also serve as an educator to patients regarding disease processes and essential follow up care related to their diagnosis.
Required Minimum Education: Bachelor's degree in related field required, Bachelor in Social Work preferred, or graduate from an accredited Licensed Practical Nurse (LPN) program with licensure as an LPN in the State of Georgia.
Required Minimum Experience: Minimum of 1 year of experience in a healthcare environment preferred
Required Minimum Skills:
Professional organizational, verbal, and written communication skills. Ability to interact appropriately with patients from various backgrounds.
