Position Summary:

The Care Coordination Assistant is part of an interdisciplinary care team consisting of Physicians, clinical professionals and social workers to assist in the cross continuum approach to hospital transitions. This position will be a key component to achieving collaborative practices that exemplify MultiCare Health Systems commitment to patient centered care, community engagement, and focus on transition of care for patients with physical and behavioral healthcare needs.

Minimum Requirements:

Experience in utilization management, community resources, and/or financial and quality issues affecting patient care desirable

Community Health Worker experience desirable

Education/Licenses/Certifications:

Post-secondary coursework in a health related field, or 1 year experience in clinical role such as medical assistant, certified nursing assistant or EMT

Bachelors degree highly desirable

Learn more at www.multicarejobs.org Position Summary: The Wound Ostomy Nurse is an integral part of the care provided to patients with wounds, stomas or fistulas of any kind. The Wound Ostomy Nurse acts as the clinical resource for Ostomy/continent diversion, skin and wound management problems, pressure ulcer prevention and incontinence issue. Plans, consults, coordinates and collaborates with staff in various disciplines, provides leadership in the delivery of care to the client/family. Consults with and provides clinical education/teaching services using advanced theory and knowledge of content to patients and staff to promote and influence positive patient outcomes. He/she will see wound and ostomy patients across the continuum of care to include emergency department, pre-operative, perioperative, postoperative, and acute or chronic concerns. He/she will receive consults from providers and nurses to provide education, training, assessment and care to these patients. The care will be provided in either the outpatient Wound Healing Centers or in the acute care hospitals working with the inpatient wound care team across the system as assigned. Minimum Requirements: Two years previous Wound and Ostomy experience Education/Licenses/Certifications: Licensed as a Registered Nurse in the State of Washington Wound certification required. Ostomy certification preferred. Will consider applicants actively pursuing Wound certification with completion within 18 months of hire. Valid drivers license and reliable vehicle or transportation Must be available to attend eight consecutive days of general orientation upon start.
