Children's Care Coordinator

Under the direct supervision of the RYC Director of Child/Adolescent Services, the Children's Coordinator will carry a caseload and provide direct care coordination to children and families who live in Clinton, Jackson, Scott, Muscatine and Cedar Counties, in Iowa. The Care Coordinator will help families connect to needed services and resources in the community. The Care Coordinator will act as the central point of coordination for families and connect together all needed treatment providers and other natural supports i.e.: JCS, HHS, Schools, Faith organizations, social service agencies.

Responsibilities
Program Level
Participate in the establishment of a single coordinated and cohesive service system for children birth to 18 years old (or still in high school).
Ensure releases are signed for all parties involved.
Deliver care coordination in the home, school, community, etc. as requested by the family and in the most convenient location to the family.
Meet with the family face-to-face weekly.
Reduce family's experience of fragmentation when accessing services.
Utilize and enhance prevention, early intervention, intervention and postvention community-based interventions.
Provide short-term community-based treatment stabilization services as needed until families are connected to community resources.
Actively participate in school, JCS, HHS, Community Organization, or hospital staffing to assure seamless services.
Provide advocacy and support for children, parents, caregivers and guardians.
Access culturally sensitive alternative supports that will coalesce around the needs of children and families.
Check in with families frequently to assure services are helpful and are being utilized.
Monitor recommended service outcomes to ensure benchmarks are being
Reports to the Director, Child/Adol Services monthly as arranged by the Director.
Ensures that productivity levels are met.
Submit weekly metrics.
Identifies and recommends improvement opportunities for children's services.
Community
Builds relationships with hospitals, community mental health centers, substance use disorder providers, JCS, HHS, AEA, DeCat, School Districts, First Responders, law enforcement, jail staff, and community providers.
Provide community education on services available through the EIR Crisis
Attends community meetings and trainings to stay current on community resources.

Qualifications
Education:
Experience:
License(s)/Certification(s):
Valid driver's license when driving any vehicle for work-related reasons.
Any required state license by degree/ education or discipline i.e., social work, counseling, nursing.
Knowledge/Skills/Abilities:
Must have interpersonal and communication skills necessary to deal with a wide variety of clients and professionals with the ability to negotiate and establish effective working relationships with a multidisciplinary healthcare and community service team.
Must demonstrate ability to identify and act upon psychosocial needs and work independently in an outcome orientated environment.
Strong leadership skills and ability to form professional working relationships with peers.
Develop, design, and track metrics and researching outcomes.
Ability to document in, have knowledge of and use of the computer.
