BJC Medical Group is a dynamic, multidisciplinary group with locations across the St. Louis metropolitan and outlying areas. The physicians, providers and staff of BJC Medical Group provide the highest level of care to thousands of patients every day.
Assists with the pro-active management of identified populations. Providse preventive health improvement services to patients identified as having high risk factors for development of chronic disease. They will also work to close identified Gaps in Care and assist in the assessment and screening of selected patient populations. Works collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective communication and monitoring of healthcare resources.
Maintains confidentiality of data in hardcopy and electronic formats. Ensures storage and achieving of completed databases.Provides support to the case management program.Completes documentation in patient record.Provides on going case management for identified patients.Provides effective case coordination and claims analysis.Provides general nursing services under supervision.BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
