POSITION SUMMARY:# Utilizing the nursing process coordinates and provides specialized cardiothoracic and pulmonary nursing care to patients in an Ambulatory Care setting.# Works collaboratively with the Cardiothoracic Surgeon/Pulmonologist to serve as a case manager and functions as a liaison between the patient and physician to provide and ensure the highest quality of care.# Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as Health system initiatives. EDUCATION/TRAINING: # Graduate from an approved School of Nursing Program.# At least one year of cardiovascular or pulmonary nursing or related experience.# # LICENSURE/CERTIFICATION: Current RN licensure to practice in the State of Illinois.# Bi-annual ACLS and BLS certification required. # EXPERIENCE/SKILLS/ABILITIES: Successful completion of annual clinical, age and job specific competencies and skill verification tolls required.# Ability to perform accurate phone triage.# Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.# Ability to direct ancillary and assistive staff and organize work assignments.# Teaching and leaderships skills essential to the position


ESSENTIAL FUNCTIONS:


Under the direction of MD or APN, provides direct patient care including assessment and history of the physical, psychological social and cultural systems of the patient.# Serves as primary case manager to the Cardiologist # Pulmonologist. In collaboration with the MD or APN, outline an individualized plan of care for each patient.# Demonstrates critical thinking skill and exercises nursing judgment in evaluating and initiating emergency measures in emergent situations following standard protocol.# Provides necessary follow-up serves for each patient ensuring compliance with outlined medical regimen.

Works collaboratively with inpatient clinicians to enhance quality of services across the continuum of care, ensuring continuity and efficiency of nursing care and coordinating necessary referrals. Identifies and implements individualized comprehensive patient and family education and counseling.
