  • Responsibilities*

  • Expectations:*

  • Ability to multitask in a fast paced environment while delivering quality patient care

  • Collect, read, and interpret assigned patient's health history

  • Create and implement care plans in collaboration with other care team members

  • Administer medications to patients and educate caregivers when necessary

  • Qualifications*

  • Minimum Qualifications:

  • Diploma in Nursing from accredited nursing program or Associates Degree in Nursing required

  • Active Registered Nurse Licensure in Arkansas or eNLC licensure with multistate privileges in good standing with the Arkansas Board of Nursing. License must be unencumbered.

  • 6 months of nursing experience required (New graduates would need to complete a residency program before consideration for this unit)

  • Basic Life Support (BLS)

  • Preferred Qualifications:

  • Bachelor's Degree in Nursing (BSN)

  • Previous nursing experience in cardiology or acute care medical surgical

(Utilization Review/Management, MDS coordinator, and Clinical Liaison experience is also strongly preferred).</p><p>4. Experience coordinating with providers, nursing and therapy staff to adjust and monitor patient care plans (Interdisciplinary Teams).</p><p>5. Solid understanding of compliance with Medicare guidelines and procedures specifically for care within skill nursing facilities.</p><p>&nbsp;</p><p>What you need to know:</p><p>&bull; The Inpatient Care Coordinator must live within a 30-minute commute of Hot Springs, AR</p><p>&bull; The Inpatient Care Coordinator is responsible for coordinating transitions and identifying appropriate care plan in order to improve patient recovery.</p><p>&bull; Coordinators will manage an average caseload of 25-30 patients</p><p>&bull; 85% of the day spent in the field providing onsite support to patients across several skilled nursing facilities (100% remote at this time due to covid).</p><p>&bull; 15% of day spent in home office-documentation, e-mails, prep (start this at end of the day).</p><p>&bull; Will have six weeks of training upon start date (virtual due to covid).</p> PWW Recruiting, Llc.
