Summary:

To provide nursing care to a patient or group of patients under the direction and supervision of a registered nurse.

Responsibilities:

1.Patient Care Criteria A: Provides patient care as directed by the Registered Nurse.

Prioritizes assignments in consultation with the RN. Patient Care Criteria B: Performs procedures as identified by the PCT Skills Checklist; refers to and utilizes appropriate resources.

Responds promptly to patient care needs. Patient Care Criteria C: Maintains a safe patient environment incorporating Norwalk Hospital Policies and Procedures.

Patient Care Criteria D: Shows initiative in noting and offering assistance with unit needs.

2.Communication Criteria A: Communicates with patients, visitors and coworkers within the scope of his/her responsibilities in a courteous manner.

Communication Criteria B: Observes and reports to Registered Nurse all changes in patient condition, observations or occurrences of an unusual nature in a clear and concise manner.

Communication Criteria C:

Documents accurately patient data on appropriate forms; i.e., Fluid Balance Record, Nursing Care Record, calorie counts, frequent vital sign sheet. Communication Criteria D: Obtains information necessary to carry out patient care assignments, for example; report and/or active order profile sheets

3.Occupational and Personal Responsibilities Criteria A: Demonstrates compliance with identified policies and procedures in carrying out unit assignments i.e., postings, work sheets, maintenance of data and knowledge of medical terminology.

Demonstrates willingness to assist with and improve unit operations. Occupational and Personal Responsibilities Criteria B: Participates in educational programs to maintain or increase job skills including but not limited to required inservices and educational programs.

Participates in quality improvement activities as requested. Occupational and Personal Responsibilities Criteria C: Maintains neat and orderly appearance and environment.

Occupational and Personal Responsibilities Criteria D: Completes all competency's and annual requirements as established by the staff development department and the nurse manager. Maintains personal educational record.

4.Equipment and supplies Criteria A: Utilizes supplies and equipment in a cost efficient and proper manner.

Equipment and supplies Criteria B: Reports problems with supplies and equipment to appropriate personnel for corrective action. Equipment and supplies Criteria C: Assures that all supplies are needed to maintain the monitor room.

5.Monitoring patient on a cardiac monitor Criteria A: Reviews patient information to determine normal heart rhythm patterns and prior dysrhythmias.

Monitoring patient on a cardiac monitor Criteria B: Reviews cardiac rhythms and analyzes the function of pacemakers and implantable cardiac defibrillators. Monitoring patient on a cardiac monitor Criteria C:

Documents on appropriate forms for the patients record, cardiac tracings, measurements and interpretations.



  1. recognition of ECG dysrhythmias Criteria A: Observes cardiac monitor screens and notifies nursing and medical staff of any dysrhythmias including changes in rhythms indicating cardiac ischemia and acute myocardial infarction. recognition of ECG dysrhythmias Criteria B:

    During code situations, maintains contact with nursing staff to assist with rhythm interpretation and dysrhythmia detection. Labels monitor strips to indicate significant events including, but not limited to, defibrillation, cardioversions, CPR, and or code medication administration.

7.Communication to nursing/medical staff Criteria A: Notifies nursing/medical staff in timely manner of ECG changes.

Communication to nursing/medical staff Criteria B: Calls code when life threatening rhythm identified. Communication to nursing/medical staff Criteria C:

Identifies and records patients rhythm at a minimum of every 8 hours and with any significant changes in patients rhythm and/or condition. Communication to nursing/medical staff Criteria D: Notifies appropriate nursing staff of pertinent data, identified information including, but not limited to, patients name, room number, cardiac rhythm, dysrhythmia, and or life threatening situations.

8.Maintenance of monitoring system Criteria A: Demonstrates correct placement of telemetry leads on patients.

Tailors patient alarms, enters new admits, discharges and transfers to the monitoring system. Assures that all information on patient, room number and nurse are obtained from all nursing units. Maintenance of monitoring system Criteria B:

Trouble shoots monitoring equipment for immediate correctable problems such as: (a) printer paper changes, (b) low battery/ out of range occurrences, (c) lead wire/ electrode placement, (d) monitor transmitter malfunctions. Keeps a log of all calls made to Philips monitoring company and biomedical engineering for issues and malfunctions. Notifies biomedical engineering immediately with problems. Changes lead wires and batteries if requested.

9.Performs other duties as assigned.

Other information:

Required: High School Diploma or equivalent is required. Experience in a Hospital setting is preferred but not required.

Minimum Experience: zero to six months

Desired: Active CNA certification in the state of Connecticut is preferred. BLS/CPR preferred.
