This job manages relationships in an assigned territory with the intent to build relationships with post-acute facilities / providers. The primary objective is to grow the services and programs identified in the assigned department(s). This job is directly responsible for the growth of assigned programs, and maintaining the quality provider image for the corporate community and associated stakeholders. Maintains relationships within the community and development of new programs to increase the organization's market share for the assigned accounts or corporate division. Promotes designated programs to our strategic partner hospitals to assist in the development of all the programs at the facilities. Provide the service line and Post-Acute network leadership with strategic feedback that will be used to analyze demands of the market, assess data and develop plans based on the department's assigned criteria or business demands.

Develops and implements partner relationships and marketing plan for assigned territories.
Meets with assigned client groups to build relationships, works with leadership to develop solutions to meet the needs of departments to expand and grow their current business while designing programs to meet the needs of clients, and to provide high level of care to patients
Participates in marketing activities to promote programs by exhibiting in events or attending events for networking opportunities
Provides the on-site promotion at identified events when necessary
Collaborates with marketing department and business leaders to design and produce collaterals including promotional items
Tracks and reports activities and business results
Stay up to date on CMS regulations for post-acute providers
Conduct monthly and quarterly Joint Operating Committee Meetings
Regular participation in departmental meetings, as required
Track and trend post-acute facility quality data on a regular basis
Complete reports, as requested by leadership
Proactively monitors hospital's post-acute admission trends and reports any negative results
Monitor territory's post-acute CMS Star Rating results
Schedule and conduct in-services for post-acute providers and hospital teams, based on needs
Monitor and address needs to post-acute providers and hospital case managers
Maintain post-acute network databases
Identify barriers and issues that could impede timely patient transfer to post-acute settings, and develop / suggest effective resolutions
Review improvement plans with post-acute facility management.
Provides education on the Post-Acute Network and benefits of being an integrated provider
Complete competitive analysis, as requested, for territory's providers
Attend business development training activities, as required, to continuously improve skills
