Positions Summary:

The Case Manager is responsible for Bridge House direct services, staffing and supervision of a transitional living program for 16-22 year old youth experiencing homelessness. The position provides group client supervision and individualized case management, including interpretation and guidance regarding program agreements; crisis intervention; intake; assessment; case planning; and progress monitoring. Programming is structured using a youth involved "self-government" model based on Community Youth Development practices. Information tracking using both computerized and paper systems and facility clean-up and maintenance routines are also part of the position's responsibilities.

Education and Experience Requirements:

  • Experience with homeless youth and/or CSEC involved youth OR experience working with local juvenile justice and youth services systems OR experience working with youth in a residential setting is required.

  • Bi-lingual Spanish language, preferred.

Required Knowledge, Skills and Abilities:

  • Genuine concern for young people and the ability to treat all people with dignity and respect, and openness to learning and implementing a Positive Youth Development approach to youth work.

  • Ability to work effectively within: culturally diverse populations; team dependent models; crisis management; strength-based theoretical orientation; adolescent development; and family reunification.

  • Ability to work effectively may be demonstrated through academic experience, job experience, references, and interview responses.

  • Knowledge of stages of change, motivational interviewing, and trauma informed care.

  • Knowledge of community resources that support and promote independent living skills.

  • Knowledge of Homeless Youth Continuum, McKinney-Vento Act, Department of Human Services, DePaul Youth Treatment Center and/or other systems of care for youth experiencing homelessness.

  • A commitment to on-going training and a willingness to receive and act upon feedback.

  • Ability to accurately read, record, and interpret information and complete written documentation that meets contract and program guidelines.

  • Ability to provide visual and auditory supervision of program and youth.

  • Ability to communicate effectively orally and in writing with a broad variety of people, including youth, families, staff, supervisors, and community partners.

  • Ability to stay calm and emotionally available in an environment serving young people who require a high level of supervision and redirection from staff.

  • Ability to exercise independent discretion and judgment within program guidelines.

  • Ability to work effectively in a team-dependent environment and to act on the agenda of the program.

  • Ability to exercise tact, discretion and judgment in working with a variety of people.

  • Ability to engage in supervision, follow program, procedural, and supervisor directions, and receive feedback in a cooperative and positive manner.

  • Ability to maintain appropriate professional boundaries working with youth and in handling confidential information.

  • Ability to climb and descend at least two flights of stairs.

  • Ability to stay awake and alert throughout entire assigned shift and to perform routine program checks and facility walk-throughs.

  • Ability to effectively monitor safety and demonstrate group management skills and effectively de-escalate potentially chaotic or volatile situations.

  • Ability to regularly attend scheduled shift, be punctual for scheduled shift and meetings, and be in a condition suitable for assuming responsibilities of position.

  • Manual dexterity allowing performance of routine office tasks such as writing, phone use, filing, copying, and use of computer keyboard.

  • Manual/physical capability to bend, stoop, kneel, and reach, as well as occasionally lifting up to 20lbs (including overhead) in order to perform functions such as accepting donations or unpacking groceries.

  • Ability to perform and/or supervise routine household tasks such as cleaning, cooking, washing and folding laundry, emptying trash, sweeping/mopping, etc.

Other Requirements:

  • Driving is not required. All employees who drive for work purposes must have and maintain a valid driver's license for state of residency, have a driving record that permits coverage under the corporate auto liability policy, and complete the agency's driving approval process before driving for work. If driving a personal vehicle for work purposes drivers must also have personal auto liability coverage ($100,000/$300,000 levels strongly recommended). Employees are not to drive for work purposes until they have received written authorization to do so.

  • Bloodborne Pathogens training is required on first day of employment and annually thereafter.

  • Must complete Mandatory Abuse Training within the first 30 days of employment and annually thereafter.

  • Provides emergency first-aid, as needed, and must have and maintain standard first-aid/CPR certification.

  • Must pass DHS criminal history background check.

  • Food Handler's Certification required within 5 days of hire.

  • TB test within 30 days of employment

Duties and Responsibilities:

The Case Manager position include the following, as well as other duties as assigned:

  • Provide a nurturing & respectful environment and supervision/security for youth in the transitional living residential facility.

  • Complete interviews with prospective residents.

  • Track residents' savings.

  • Manage interactions and crisis situations constructively and within program policies and guidelines.

  • Ensure the physical and emotional security of youth in the programs.

  • Inventory and write house supply lists.

  • Shop for program supplies.

  • Assist in staff training regarding relevant house / resident issues.

  • Provide follow-up services to former Bridge House residents.

  • Sort and file mail.

  • Provide on-going support, as needed.

  • Refer to Changes for housing needs and in-depth case management concerns.

  • Support and monitor the implementation of program agreements with youth through individualized interventions and, if indicated, follow-through with consequences.

  • Provide individualized case planning, support, and related services through formal case management meetings and intake/exit processes.

  • Provide routine maintenance of facilities, including cleaning responsibilities, safety and/or security projects, and other duties to ensure compliance with regulations and standards.

  • Communicate maintenance requests with program supervisor and document in Staff log.

  • Run monthly fire drills according to program policies.

  • Generate and maintain relationships with other youth service programs.

  • Attend and participate in staff meetings, supervisions, coordination meetings and other meetings, including but not limited to trainings and retreats. Represent the program at community meetings / functions as assigned or required.

  • Perform assigned paperwork and electronic record keeping duties according to program policies, guidelines, and/or supervisory instruction in a timely manner.

  • Support and assist substitute and temporary workers and volunteers.

  • Model and encourage direct, professional communication within the program and community, and facilitate development of a positive working environment.

  • Maintain appropriate boundaries and adhere to policies and guidelines established in the agency employee guidebook and Willamette Bridge Program's service provision policies.

  • Provide emergency first-aid as-needed and maintain current standard first-aid certification.

  • Performs other duties as assigned by Supervisor, Program Director, or Executive Director.

Working Conditions:

  • May involve exposure to communicable diseases, including a variety of illnesses and infections such as the common cold, TB, Hepatitis, flu, meningitis, and HIV.

  • May involve exposure to upset, angry, severely traumatized or emotionally disturbed children, adolescents, adults, and families.

  • May involve time in excess of routine schedule and overtime, including various shifts and/or weekend hours, and/or holidays.

  • May involve exposure to information that graphically describes physical, sexual and emotion abuse of children and may experience vicarious trauma.

  • May require working and providing services at locations other than program site (i.e., schools, café', clinics, etc.,) and may, therefore, be exposed to environments not under agency control (i.e., second-hand smoke, varying hygiene practices, etc.).
