Breast Health Nurse Navigator

4 - 9 hour shifts (some flexibility) between the hours of 7am
5pm. No Weekends/Holidays

Additional Job Duties:
Dedicated to fulfilling the navigation function of system-wide breast health for BiRad 0,3,4 and 5, dense breast, and identified intermediate or high risk for breast cancer patients.
Build a program under the direction of the Henry Ford Health
Detailed Job Duties:
o Follow up all BiRad 0,3,4 and 5 results for breast imaging in a coordinated effort with the patient and provider.
o Obtain follow up orders for BiRad 0,3,4 and 5 to ensure they are completed and navigate the patient according to the results per the BiRad and Dense Breast Workflow Algorithm (attached).
o Review and protocol all non-screening breast imaging and procedure orders.
o Scan into EPIC orders for follow up based on BiRad list when received from independent providers.
o Request prior outside breast imaging films and reports.
o Manage the work queue for scheduling breast imaging and procedures.
o Make first patient contact to schedule breast imaging and procedures, except screening mammogram.
o Schedule recommended surgical consult appointments, coordinating next available with surgeons' office, and notify patient of appointment.
o Call patients who have benign biopsy results, answer questions, and educate patients, staff, and providers appropriate to breast health.
o Call all identified high risk screening mammogram patients with information and schedule them for an appointment at the High Risk Breast Clinic and other follow up based on the Radiologist recommendation(s) and High Risk Breast Workflow Algorithm.
o Manage the High Risk Breast Clinic referral work queue.
o Track patients for BiRad 0, 3,4, and 5, dense breasts, and Intermediate and High-Risk patients until discharged from High-Risk Program. Document as appropriate in Epic EMR.
o Refer confirmed positive for breast cancer patients to the Oncology (Breast Cancer) Nurse Navigator.
o Attend the Breast Health Council and Breast Site Disease Team meetings and report data, findings, and barriers to care.

Job Description:
Nurse Navigators are nurses with specialty-specific clinical knowledge who offer individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers across the continuum of care. Navigators Collaborate with other health care team members including physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, patient access coordinators, and ancillary and other support staff, and referring physicians to create a high-quality, efficient, safe, and exceptional patient experience.
EDUCATION/EXPERIENCE REQUIRED:
Graduate from an accredited school of nursing required.
Bachelor's Degree preferred.
Two (2) years of nursing experience required.
Specialty experience preferred.
Maintains and demonstrates current clinical knowledge in area of specialty.
Strong organizational, leadership, listening, critical thinking, problem solving, and interpersonal skills.
Strong communication skills, written and verbal.
Ability to prioritize and reprioritize quickly.
Ability to develop collaborative relationships both internally and externally.
Ability to work in teams and in a self-directed, autonomous manner.
Able to delegate and coordinate efforts of the team/co-workers.

Understands Musculoskeletal Health specialties, pathways and procedures, goals of care, symptom management, quality of life, and evidence-based practice guidelines.

Takes necessary actions to develop and maintain personal job competencies/expertise and to improve performance on an ongoing basis; keeps current regarding medical and technological trends in healthcare overall and/or specialty/service line best practices, as well as a working knowledge of insurance rules and community resources.
Computer skills and knowledge of Windows, Outlook and electronic medical record.
CERTIFICATIONS/LICENSURES REQUIRED:
Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
Specialty-specific certification preferred and desired within two (2) years of hire as appropriate to job being filled.
NOTE: The Nurse Navigators for the Musculoskeletal Health Service Line already employed by HFHS orthopedic services have two (2) years to complete certification after entry into this job.
