Summary

Reporting to the Manager or Clinical Leader/Manager, and under the direction and supervision of the medical provider, assists in the examination and treatment of patients and has advanced skills in immunization administration, treatments and procedures. In addition, may be involved in other areas of medical office support including clerical, environmental and organizational.

Education

Some additional training beyond high school, but less than an Associates Degree. Completion of a Medical Assistant training program preferred.

Experience

Minimum of six months related experience and/or training in similar medical practice/field.

Education Graduate of an accredited nursing program. BSN required. Certification, Registration & Licensure Active, unencumbered NH Board of Nursing RN license. Certification in area of specialty highly desired Required Upon Hire: Current BLS for Health Care Provider certification. Experience Minimum three years nursing experience; case management and/or care coordination experience preferred. Outpatient or medical office experience highly desired. Experience with assigned patient population preferred. Responsibilities * Coordinates the care provided by the health care team for assigned patient population and provides care coordination with other disciplines and across other health care organizations and entities in collaboration with multidisciplinary team. * Provides informal and formal education to patients, FHC Clinical Staff and Family Medicine Residents. * Demonstrates expertise in all aspects of nursing care for assigned patient population, serving as a consultant and a provider of telephonic and direct patient care as needed. * Develops, in collaboration with providers and the health care team, population and patient specific plans of care to address identified health care needs. * Collects, reports and provides analysis of data relative to assigned patient population. * Serves as the primary contact and liaison for community, State and Federal partners related to assigned patient population, as needed, ensuring compliance with regulations and reporting expectations. * Arranges for and follows-up on diagnostic testing, referrals and other follow-up care ordered for individual patients within assigned patient population and in collaboration with the health care team. * Applies principles of purposeful, organized thinking based on the nursing process to make sound clinical judgment and decisions.
