Fellowship Health Resources affiliated with Elwyn in 2018 and provides behavioral health services to improve the quality of life for individuals living with mental illness and addictions.

A Blended Case Manager works with Adults 18+ with serious and persistent mental illness to improve their overall functioning and quality of life. This may include securing benefits/insurance, locating and maintaining affordable housing, mental health stabilization/crisis management, physical health coaching, seeking vocation/education opportunities, and linking to community resources to overall meet their needs and goal areas.

1.GENERAL RESPONSIBILITIES:

Assist Program clients in general daily care. Provides counseling and advocacy in therapeutic setting assisting young adult through geriatric clients with mental illness achieve social, economic, emotional, and personal independence.

2.REQUIRED QUALIFICATIONS WITH REASONABLE ACCOMMODATION:

Pre-employment Education

A bachelor's degree in Human Service field, such as sociology, social work, psychology, political science, history, criminal justice, theology, counseling, or education is required.

Pre-employment Experience

A commitment to community-based mental health services. Prior experience working with the mentally ill adult population preferred.

4.ESSENTIAL FUNCTIONS AND PERFORMANCE STANDARDS:

A. Clinical Skills:

  • Understands the signs and symptoms of mental illness and intervenes therapeutically.

  • Demonstrates knowledge of prescribed medications and ability to monitor medications.

  • Practices non-violent crisis intervention.

  • Demonstrates competency in the development and implementation of treatment plans as evidenced by:

  • Delivers interventions of resource linking, resource modification and development, skill teaching and programming to clients. Includes, but is not limited to: assists clients in getting financial benefits; assists clients in accessing medical and dental care as needed; assists clients in choosing, getting, and keeping employment; assists clients in choosing, getting, and keeping quality, affordable housing; assists clients in developing social support network and participating in social and leisure activities; and assists clients in applying ADL skills in real-life environments.

  • Ensures interventions and supports are delivered in a way that incorporates consistency, flexibility, and responsiveness to changing client needs.

  • Involves clients in identifying and coordinating treatment issues.

  • Empowers clients through therapeutic rapport, advocacy, and skill

building.

  • Exhibits proficiency with benefit and entitlement options to maximize

treatment planning opportunities.

  • Coordinates emergency psychiatric services within a timely manner,

including documenting interventions, alternatives considered, and

response of client/client family, and supporting agencies.

  • Provides, coordinates and monitors treatment services for all assigned

cases. Includes, but is not limited to: documenting face-to-face and

telephone contact with client and provision and coordination of care

prescribed in the Individual Treatment Plan. Implementing assigned

activities with other client/client family, FHR staff, consultants, and

treatment plan designated staff from other human service, medical

organizations and schools. Monitoring the effectiveness of care

provided through other individuals and agencies, by comparing

services provided with service outcome, as specified in the treatment

plan.

  • Understands clients baseline level of functioning and is able to

recognize significant deviations as a possible sign of decompensation.

Reports signs of decompensation immediately to supervisor.

  • Knowledgeable of DSM-IV classifications and conditions.

  • Participates in treatment planning by: completing assessment

assignments within time requirements specified by procedures;

scheduling/attending treatment planning meetings with staff and other

individuals specified in procedures; utilizing assessment information,

diagnostic results and staff participation in the development of

treatment plans; and completing assessment, treatment plan and

reviewing documents following time and other technical

specifications.

  • Provides psychoeducational services to clients and their families/

significant others based on modality, frequency, participant and

location specifications prescribed in the Individual Treatment Plan.

Services will include provision of information regarding techniques

for managing symptoms and stressors, education and training

regarding psychotropic medications, skills training regarding co-occurring disorders such as substance abuse (if indicated), training and support regarding activities of daily living, training and support regarding communication, social and recreational skills; and skills and

knowledge required to transition to independent living.

  • When providing therapeutic/counseling/case management services,

arrives and/or participates on time, as scheduled; is alert and

responsive to client issues and behaviors; works within prescriptive

instructions from the client Individual Treatment Plan, modifies

treatment planning to meet client needs and changes in situation; and

documents, with the client if possible, treatment implementation and

related issues in progress notes and/or treatment plan review

documents.

  • Assists in client Discharge Planning.

  • Case record tasks will be performed following agency procedures,

including: updating of client database; reviewing outside

correspondence; and following client confidentiality and client rights

procedures.

  • Makes arrangements with other agencies to provide services for

identified clients as specified in client treatment plans.

  • Establishes and maintains effective therapeutic relationships.

  • Promotes group process, involvement, and community integration.

  • Fosters positive relationships with both primary customers (those who

use our service) and supporting customers (members of the

community, regulatory agencies, etc.)

  • Assists in transportation planning.

  • Integrates the Prism Model for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.

  • B. Program Operations:

  • Practices Fellowship policies and procedures congruent with Organization mission.

  • Participates in performance improvement activities.

  • Organizes, prioritizes, and completes multiple job-related responsibilities.

  • Maintains client records.

  • Participates in and attends staff meetings.

  • Produces clear, timely, thorough, and accurate record-keeping in compliance with Program, Department, County, and OSHA regulations on daily or as needed basis.

  • Attends scheduled supervision with ICM Supervisor.

  • Demonstrates all employment actions are carried out in a safe manner, rendering a safe environment for all clients, visitors, and employees.

  • Notifies supervisor of any pertinent information to ensure quality client care.

  • C. Communication Skills:

  • Verbally communicates in a manner easily understood by the listener.

  • Writes proficiently: clearly communicates relevant information and uses correct grammar, spelling, and punctuation.

  • D. Teamwork:

  • Establishes positive working relationships and promotes strong team morale.

  • Demonstrates flexibility and adaptability.

  • Resolves conflict diplomatically.

  • Performs all job-related duties and individual performance goals as assigned by ICM Supervisor.

  • E. Professional Development:

  • Participates in regular supervision, welcomes feedback, and implements action plans.

  • Sets goals and works to achieve them.

  • Regularly reads FHR emails to stay abreast of pertinent work information.

5.WORKING CONDITIONS:

Necessary Travel

Driving required in the course of duties. Valid state driver's license and record in good standing. Driving in inclement weather as needed. Use of personal vehicle may be required.

Unusual Work Hours

Hours as assigned by Program. Scheduled holiday and severe weather attendance may be required. On-call responsibility as needed.

Unusual Environmental Conditions

Work area - inside and outside. Environment may be noisy at times. Hazards - exposure to infectious disease. Necessity to practice universal precautions at all times.
