Assist Home Care Staff with administrative and consumer related functions to support the work of Care Managers and Nurses, including home visits to applicants/consumers as required. Also assist Home Care Staff with interpreting for non-English speaking or limited-English speaking consumers on the telephone, in the office and in consumers' homes.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
Tasks will include but are not limited to:
Assist Team by taking and making telephone calls to English speaking and non-English speaking or limited-English speaking consumers or family members to schedule appointments, follow-up on consumers on the Wait List or gather information as directed by the Nurse or Care Manager and completing limited paperwork/consumer documentation as needed.
Assist Care Managers with follow-up consumer work including service suspensions/resumptions, procurement of services, accessing durable medical equipment for consumers, locating consumers who were not at home to receive services, etc. and completing consumer documentation and data entry as required.
Assist consumers to apply for MassHealth and complete annual MassHealth renewal paperwork including making home visit to the consumer, assisting consumer to complete application/ required paperwork, obtaining and making copies of required verifications, etc.
Assist consumers who are requesting Elder Care Funds for emergency purchases such as prescriptions, food or other necessary items by procuring and delivering required items to the consumer.
Perform administrative functions to support the Team including completing standard forms (on paper or on line) including PT-1 Transportation forms, Rental Housing applications, PERS applications, etc. as well as filing, copying, etc.
Accompany Nurse, Care Manager, Community Liaison or other Home Care staff on consumer home visits to interpret information in English as required for determination of eligibility, assessment of needs, development of a care plan and problem solving/crisis intervention.
Translate forms, letters and other documents into other language of fluency as required
Observe all confidentiality regulations and protect consumer information.
Attend mandatory meetings and trainings including Team Meeting, General Staff Meeting and Home Care Staff Meeting.
Other duties as assigned by Home Care Management Staff. Associates Degree and experience as a medical interpreter preferred. High School diploma, fluency in both required language and English and a minimum of one year of related work experience required. Experience in Human Service environment helpful. Computer skills required. Must be responsible and have compassion for older people. Excellent interpersonal and communication skills a must in this team oriented position.
