Responsibilities

As a healthcare and business professional and representative of Client, the On-Call Specialist will be expected to:

  • Work on an "as needed" basis

  • Provide disease state education on lipodystrophy and visceral adiposity

  • Communicate identified patient feedback to client

  • The Services provided by On-Call Specialists shall only be utilizing approved client materials

  • Present himself or herself as a business professional

  • Perform all activities in a manner which demonstrates a command of customer service principles and adult learning styles

  • Provide documentation and written feedback in a timely and comprehensive manner as required

  • Maintain any equipment which has been provided by Client, as demonstrator equipment or otherwise, reasonable wear and tear expected.

  • Deployed through Schedule 360 via the Theratechnologies KAM

  • Programs are mostly in the day time hours (morning or afternoon) but may be in the evening hours and weekends , the ideal candidate should be available both day and night and on the weekend

  • There may also be some patient advocacy "speaker" engagements

Job Requirements

Along with a results-oriented mindset, self-assurance and an engaging manner, the ideal candidate has:

  • Must have minimum of RN, BSN preferred

  • Active Registered Nurse Licensure

  • 2 years clinical experience in HIV Therapy

  • Excellent presentation skills

  • Able to travel if needed by flight

  • Commitment to availability for up to 4 assignments per month

  • Preferred understanding of ASOs ( Aids Service organizations) and the HIV community

Who You'll Be Working With: Evolent Health is actively recruiting for a Health Educator to join a team of clinical colleagues working together to improve the health of a defined population. This position will be focused on moderate risk/at risk patients living with chronic disease(s). The Health Educator will assist our patients and caregivers in finding new ways to inspire healthier behaviors and build the self-management skills of our patients and care givers. The individual taking this role will manage his/her caseload through telephonic and electronic communications and coordination. What You'll be Doing: * Conducts initial outreach calls to encourage patients and care givers to participate in the program * Contacts and performs initial interviews/screenings with eligible patients. * Provides self-management and care coordination support for patients with one of the following conditions: Asthma, Chronic Obstructive Pulmonary Disease, Coronary Artery Disease and Diabetes. * Assists the patient and care giver to understand their illness, make life style changes, adhere to medication regimen and work with their provider to develop a symptom response plan * Creates a personalized action plan that will be reviewed with an RN Care Advisor. The action plan will include short and long term goals prioritized by the patient, specific activities/interventions to achieve goals. Identify and address barriers that are preventing the patient from meeting their goals. * Conducts regularly scheduled, generally every 3 weeks, self-management coaching, education calls about the patient's condition and role in taking care of themselves; this includes care coordination with patients, care givers, physicians and other community and health plan based resources to ensure patients are receiving the right care in the right place at the right time; * Arranges referrals to Evolent extended care team members including registered nurses, social workers and pharmacist, as well as, network/delivery system providers and services; * Documents their activities in the Identifi Care system, including action plan and activities conducted * Educates the patient and/or care giver about symptom response plans * Arranges transportation * Facilitates gaps in care closure by educating patients about preventive monitoring/screening and working with physician practices to schedule diagnostic testing * Adheres to documentation standards and policies and procedures; including frequency of outreach * Understands and supports Evolent and Client goals, including program metrics, improvements in quality, cost of care, and member experience for the population that is managed. * Identifies opportunities for improvement (at individual, client and program levels) and actively collaborates with management and the broader Evolent clinical team to achieve and/or improve results The Experience You'll Need (Required): * Two years of experience working with commercial, Medicare and/or Medicaid chronic populations to provide health education, self-management, wellness, care coordination support and lifestyle related topics. * Experience working in a health plan setting or doing predominantly telephonic health education and coaching is not required but it is highly preferred * Respiratory Therapist, Registered Dietitian, RN or LPN with B.S. in Health Education, Health promotion, Community Health Education or equivalent education. * Ability to connect with people with diverse needs and background through the telephone * Bilingual in either Spanish or Creole Finishing Touches (Preferred): * Health Coach Certification completed prior to or within the first two years of employment at Evolent * Demonstrates innovation and an acceptance of new ideas and concepts * Possesses the ability to engage/motivate patients to make vital changes in their lives * Proven skill in making necessary linkages in order to meet specific needs * Must have strong organizational (time management) skills, strong interpersonal skills and the ability to handle multiple priorities * Knowledge of and practical use of good business English, spelling, arithmetic, practices and the ability to communicate effectively using written and verbal skills.
