Geriatric Support Services Coordinator (GSSC)

This is a Local Union 509 full-time, non-exempt (35 hour/week) position. We offer a hybrid remote schedule (1 days/week in the office) and the option of a flexible 4-day work week upon completion of an introductory period. Our office is located in Malden, MA with ample free parking!

Compensation: Starting annual salary is $47,274.86 Being bilingual is a requirement for this role and we recognize it as an asset and offer an additional 6% bilingual designation. Enjoy benefits that start as of day 1 of employment and our generous paid time off package.

In this role you will be responsible for assessing consumers for need and eligibility for in-home services, recommending/approving service plans, providing information on available resources, and coordinating in-home/community services. You will also work collaboratively with Senior Care Options (SCO) coordinators and nurses to provide comprehensive service planning for the older adults in the community setting. The GSSC works with SCO plans including Commonwealth Care Alliance, Tufts Health Plan, and Senior Whole Health.

What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

  • Conduct initial and on-going assessment of consumers' health and functional status.

  • Conduct visits as required in various settings including consumers' homes, community settings, and/or medical facilities.

  • Participate in development of consumers' care plans, including determining appropriateness and need for in-home/community-based services.

  • Participate as part of consumer's primary care team including, attending team meetings, trainings, and joint home visits.

  • Arrange, coordinate, and oversee in-home and community-based services for consumers.

  • Monitor consumer's health status, in conjunction with care team, to assess for effectiveness of current services, need for additional services, and/or need for long-term placement.

  • Assist consumers with MassHealth redeterminations and other benefits paperwork, as requested and appropriate.

  • Maintain consumer records in accordance with standards.

  • Compile and submit required reports.

Qualifications:

  • Bilingual Haitian Creole/English

  • A Bachelor's degree in Social Work or a related field.

  • Social Work license preferred.

  • Knowledge of and experience working with older adults in a community-based setting preferred.

  • Strong interviewing and assessment skills.

  • Strong interpersonal and organizational skills.

  • Excellent written and verbal communication skills.

  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches

  • Private transportation.
