Boulder County Public Health is seeking to hire a BilingualFamily Connects Public Health Nurse. The public health nurse position will perform maternal child health nursing functions for the early postpartum dyad under the Family Connects Program at Boulder County Public Health (BCPH). The nurse will conduct home visiting for families who give birth at Boulder Community Health in Boulder, CO, as part of the Family Connects Program.

The position goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Family Health Nurse will work in a team environment of clinical and non-clinical staff in order to schedule, visit, and support families in the community under the Family Connects model and the Children with Special Needs Program: NICU-to-home support.

These areterm limited position with an anticipated end dates ofDecember 25, 2024. Ongoing funding is likely beyond the term end date, but not guaranteed. The work schedule is Monday

  • Friday, 8:00am

  • 4:30pm with some flexibility. These positions will workout of the St.

    Vrain Hub- 515 Coffman Street, Longmont Colorado ; field work may occur throughout Boulder County. Under FLSA guidelines, this position is exempt.

Boulder County requires its employees to reside in the state of Colorado as of the first day of work.

Bilingual Public Health Nurse I Hiring Salary Range: $60,756.00 - $74,136.00Annually

Bilingual Public Health Nurse II Hiring Salary Range:$70,320.00 - $85,794.00 Annually

TENTATIVE HIRING TIMELINE:

  • Phone Screening: held weekly until filled

  • First Round Interviews: held weekly until filled

  • Second Round Interviews: held weekly until filled

New employees receive an80-hour bank of vacation at the time of hire, in addition to 8 hours of both vacation and medical leave accruals each month. Boulder County offers bountiful benefits, including pension contributions.

Examples of Duties

1.Attend the 3-day training on Family Connects home-visiting protocol. Conduct in-home assessments of infant and caregiver(s), per protocol.

Keep records of client observations and activities, as required. Document information for family assessments and program evaluation

2.Provide a health and physical assessment of mother and baby in the home to determine well-being

3.Assess family needs in 12 domains of physical and psychosocial wellbeing, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions

4.Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan with goals, objectives, and activities that address family strengths and needs

5.Develop individual health care plans with infant/caregiver dyads, families, and communities. Provide health care instructions, and anticipatory guidance

6.Work with the program team to ensure appropriate referrals and connections to community resources, as needed

7.Work in a team to assist parents in making and attending appointments for doctors, social service agencies, etc. Act as a liaison between families and other community agencies, serving as family advocate, as required

8.Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family

9.Apply effective time-management skills, organization, and prioritization skills Documentation should be completed in compliance with state guidelines, ideally within 24 hours of the encounter



  1. Work cooperatively as an active member of the team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings, as required. Support and encourage development of other staff

  2. Participate in multi-disciplinary teams and committees, consult, and coordinate with other health care providers, and refer clients to other agencies, as needed

  3. Support and collaborate on the promotion and integration of all Public Health programs, including but not limited to the Children with Special Needs and the Family Connects programs

  4. Performs related work as required

  5. May be reassigned in an emergency situation (e.g., wildfires, floods, disease outbreaks, etc.)
