What you bring to SBHG (Qualifications):
Master's degree in a social science field required, plus two (2) years of experience working in a behavioral health field.
Bachelor's degree in social science field required, plus (4) years of experience working in a behavioral health field.
Associate degree in social science field required, plus (6) years of experience working in a behavioral health field.
Level 42: BBS registration as MFT-Intern, ASW, PCCI, or Psychology Assistant; BBS licensed as LMFT, LCSW, LPCC, or Psychologist, preferred.
Valid California Driver's License
Meets Agency driver eligibility criteria
How you will make a difference (Job Overview):
This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the ECM qualifying youth.
Division/Program Overview:
The primary role at Capital Star involves facilitating ECM services for eligible TAY youth, acting as a liaison between the Division of Behavioral Health and subcontracted providers. Responsibilities include coordinating in-person visits, advocating for youth with healthcare professionals, connecting them to community services, and providing crisis intervention when necessary. The position requires a versatile approach, delivering services in various settings and using personal vehicles for offsite locations. Additionally, the role involves documenting all services in the Electronic Health Record, ensuring adherence to ECM services, and incorporating TIP, TIC, and Motivational Interviewing into service delivery. Outcome measures and reports are completed in collaboration with the Clinical Supervisor and QA/Training.
