Position Summary:

CHRIS 180 Migrant Wellness Program provides 24-hour care for unaccompanied children (UC) under the care of the Office of Refugee Resettlement (ORR) who are placed in residential care as they prepare for sponsorship. The program provides stabilization, behavioral support, comprehensive case management, educational services, daily living skills acquisition, recreational and therapeutic activities, and coordination of care. The Migrant Wellness Program provides a family-involved, trauma-informed, and strengths-based approach to care within residential communities throughout the Atlanta area.

This position provides coordination of children and/or adolescents in placement with a wide range of responsibilities for documentation, case coordination, and coaching. Because of the life experiences and trauma histories that our residents bring to their placement here, staff must not only provide adequate safety and supervision, but must be actively engaged in the process of building relationships, understanding, and solving problems as part of a team, and must offer energy, safety and commitment in their work with our residents through genuine, caring and empathic engagement.

Role and Responsibilities:

The primary responsibilities include but are not limited to the following:

  • Provide positive role-modeling to consumers/clients/members/residents

  • Promote self-esteem

  • Promote healthy boundaries

  • Promote good hygiene and appropriate personal appearance among youth and young adults

  • Reinforce socially acceptable behaviors (i.e., manners, eating habits, safety)

  • Teach and reinforce thoughtful decision making

  • Assess residents history of trauma, strengths and needs

  • Administer Ansell Casey Life Skills Assessment (ACLSA)

  • Aware of ACE score and implications on service planning

  • Develop and monitor Individualized Service Plan

  • Develop and update Life Skills Plans

  • Coordinating Services and Collaboration

  • Participate in Team Meetings for all residents on caseload with resident as available,

  • residential staff and community stakeholders, including custodians and referral or funding

  • sources as appropriate

  • Coordinate and facilitate Service Plan reviews as a part of the above Service

  • Team Meetings. When case managers, custodians, and referral sources cannot attend the meeting to provide direct input, solicit input via telephone and assure written follow up to provide content of the meeting Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to residents

  • Collaborate and consult with other providers and entities in the provision of best care services to consumers

  • Monitor the provision of services provided

  • Coordinate discharge planning to facilitate discharge plans

  • Specific services to program operations

  • Take part in staffing referrals

  • Maintain a working office in at least one group home and a work area in the other. Visit each home routinely and act as a second set of eyes to assure compliance with organizational standards.

  • Ensure that all placements, discharges, and transfers are done in partnership with the custodian and/or funding source

  • Review the intake packet and disseminate safety and service plan information from the Intake department to residential staff

  • Assure compliance with the custodian agencys case plan

  • Documentation Requirements

  • Develop the Assessment Summary based on the Intake information, the ACLSA and staff observations within 30 days

  • Develop the comprehensive individualized RBWO service plan for each youth based on assessed needs as well as youth and custodian input within 30days of admission and quarterly thereafter unless sooner if indicated based on a major change or presenting issue

  • Write discharge summaries for each youth upon day of discharge (14 days for emergency discharges) and provide to central office in order to mail to the custodian

  • Performance and Conduct

  • Maintain confidentiality of consumer information

  • Maintain appropriate professional boundaries

  • Works on other duties, tasks and projects as assigned

  • Ensures agency compliance with National Council on Accreditation and other mandatory regulations/standards

  • Lives and works by the CHRIS 180 Values and Code of Ethics

  • Follow mandated reporting requirements of GA Code

  • Support and maintain an environment of emotional and physical safety for consumers, staff and other stakeholders at all times

  • Demonstrate customer service

  • Promote positive image of CHRIS 180 programs at all times

  • Work as a contributing and collaborative team member

  • Respond to consumer and peer needs in a timely fashion

  • Answer telephone calls and emails accordingly

  • Respond to requests for information from stakeholders in a timely fashion

  • Maintain Personal Information and Training

  • Attend and participate in staff meetings as scheduled

  • Attend and participate in required trainings as scheduled

  • Maintain required certifications

  • Review, understand and responsible to program policies

  • Provide documents in a timely fashion to Human Resources

Qualifications/Competencies:

  • Must be bilingual in English and Spanish

  • Bachelors degree required (Masters preferred) in Social Work, Psychology, Education Counseling or related social service field

  • At least two (2) years of relevant health care, behavioral health or community-based experience working with women families, and/or children

  • Possess working knowledge of the behaviors and characteristics of unaccompanied youth.

  • Ability to exercise good judgment when responding to referral sources

  • Experience and ability to work well under pressure with the possibility to intake two to three youths in one day

  • Demonstrated ability to work well under pressure, handle multiple projects simultaneously, and prioritize and manage work under tight deadlines

  • Discretion is a must; demonstrated strong professional ethic is required in order to deal with confidential and sensitive matters

  • Proven computer skills are required including Microsoft Word and Excel

Schedule: On-Site

The hours will vary from week to week with a maximum of 40 hours a week at any given time unless written permission is given otherwise. The schedule will include weekdays, weekends, and holidays.

Salary: $48,000.00/annually
