Ensures resident files are complete by:

  • Creating a resident file upon acceptance of a new resident

  • Maintaining files in accordance with policy including

  • Tracking missing initial documentation (including medical permissions/authorizations)

  • Requesting missing initial intake documentation as needed from referring source or parent/guardian

  • Randomly auditing files to detect compliance issues

  • Processing incident reports daily by entering on JAIS/MiSACWIS or other electronic systems or sending to referral source

  • Filing documentation as needed

Supports Clinical Team by:

  • Entering new residents on the tracking sheet for the assigned therapist including relevant due dates

  • Tracking pending due dates for clinicians as needed

  • Assisting with/Conducting didactic group as needed

  • Updating clinical information received on potential referrals in Salesforce

  • Obtaining school and nursing records for clinical chart and filing in designated area of chart (progress reports, Ansell Casey, Psychiatric Evaluations, Medication Reviews, etc.)

  • Update resident contact list and coordinate approvals for family members on list via email

  • Collects all requests for transportation services from therapists and nursing department and coordinates with Apex Program Director to ensure adequate staffing and appropriate scheduling.

  • Delivering court reports to local courts as needed

  • Attending court hearings as needed/appropriate

Coordinate Resident Intakes by:

  • Reviewing the resident packet and if documents are missing (i.e birth certificate, etc.) prepares and sends out a request for documentation to the CMO/DHS/referral source worker

  • Preparing for intake (intake packet, clothing, shoes with name on them, hygiene materials)

  • Preparing face sheet / updating HMS status once youth is admitted

  • Distributing face sheet to: Quality, appropriate Therapist, Educational Director, Program Director, Office Manager, and the nursing department

  • Distribute necessary intake documents to Business Office Director, nursing department and education department.

  • Providing the assigned Therapist with the clinical intake packet and completed clinical chart

  • Preparing an approved contact sheet for the resident under the direction of the Therapist and places a copy in the Command Center's contact list binder

Assists in facilitating resident interaction with family by:

  • Maintaining a list of family visitors approved by Therapists

  • Scheduling family visits and maintaining a calendar to record all visits

  • Coordinating home passes with referring agency as approved by therapists

  • Scheduling, monitoring and documenting resident phone calls as approved by Therapists

Coordinates resident movement by:

  • Maintaining a calendar of resident court dates and coordinating with transportation

  • Discharging the resident off of the electronic system

Ensures coordination of services by:

  • Participating in clinical team meetings

  • Interacting with the Quality Department as required

  • Interacting with Nursing Department as directed

  • Interacting with program staff as directed

  • Interacting with representatives from other agencies as directed

  • Actively participating in team concept approach
