Position Summary:

CHRIS 180 Migrant Wellness Program provides 24-hour care for unaccompanied children (UC) under the care of the Office of Refugee Resettlement (ORR) who are placed in residential care as they prepare for sponsorship. The program provides stabilization, behavioral support, comprehensive case management, educational services, daily living skills acquisition, recreational and therapeutic activities, and coordination of care. The Migrant Wellness Program provides a family-involved, trauma-informed, and strengths-based approach to care within residential communities throughout the Atlanta area.

The Case Manager is responsible for helping children and families with the reunification process by providing casework and supportive services to children in foster care, which includes communicating with and assessing any family members identified or referred for potential sponsorship and assisting with sponsorship applications as needed. The Case Manager is responsible for the intake, assessment, case planning, service coordination and monitoring, and discharge of the Unaccompanied Children referred to CHRIS 180. In addition to casework and supportive services, the Case Manager will also be responsible for communicating with stakeholders in an effective and timely manner to ensure case continuity.

Role and Responsibilities:

The primary responsibilities include but are not limited to the following:

  • Ensure that all assigned youth on your caseload and under your supervision receive mandatory services while in care to include but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with the time and documentation expectations of the program.

  • Develop and complete assessments, service and treatment plans, progress notes, incident reports and any other necessary documentation in a timely manner.

  • Create a trusting and safe relationship with the client and encourage personal growth of the client.

  • Maintain accurate and current information on children in care, while ensuring ORRs, LIRSs and the programs documentation practices and reporting requirements are met.

  • Actively participate in weekly staff meetings with the clinical team and stakeholders.

  • Attend weekly meetings with supervisor to consult on cases, to review case plan decisions, and to evaluate client participation and progress.

  • Maintain effective communication with community agencies or individuals involved in service provision.

  • Attend internal and external training and apply newly gained knowledge in providing services.

  • Attend and prepare for required court hearings (if applicable).

  • Transport clients on an as needed basis.

  • Explore potential community resources and refer UC and families as needed.

  • Maintain updated, working knowledge of and adhere to federal (ORR/UC/DHS), state, program and agency regulations, policy and operating procedures.

  • Have ability to work flexible hours and work independently within the field.

  • Provide translation support as needed for clients, sponsors, and staff.

  • Comply with laws and regulations regarding client confidentiality.

  • Perform other duties as assigned.

Qualifications/Competencies:

  • Must be bilingual in English and Spanish

  • Bachelors degree in Human Services, Social Work or related field of study from an accredited college

  • At least one (1) year of child welfare case management experience

  • Bilingual in Spanish and English.

  • Ability to handle a maximum caseload of 8 clients.

  • Must be 21 years of age and pass a criminal history screen, including state and local child protection agency registries.

  • Excellent organizational and time management skills

  • Ability to collaborate with others, including case management staff, educational team members, supervisors, and external stakeholders.

  • Ability to practice cultural sensitivity and work effectively with diverse populations

  • Ability to be flexible with working hours and on-call responsibilities

  • Excellent writing and communication skills

  • Computer skills adequate to perform essential functions (using the UC Portal, Microsoft Office, and any internal case management software)

Schedule: On-Site

The hours will vary from week to week with a maximum of 40 hours a week at any given time unless written permission is given otherwise. The schedule will include weekdays, weekends, and holidays.

Salary: $57,000.00/annually & $3,000.00 New Hire Sign-On Bonus
