Our Youth & Family Services program in Woodburn is hiring a Care Coordinator to join the team.  The prospective candidate will be working in our Wraparound Program to help orient youth, families, and community partners to the Wraparound team approach, and help families develop individual care plans with the services and supports to meet their needs.

Wraparound is guided by 10 principles, family voice & choice, strengths based, team based to name a few, and we are looking for someone who shares these principles and values. The candidate will also provide safety planning, case management to assist youth/families with access to resources and supports and collaborate with community partners for coordination of care. Wages:

Care Coordinator:  $28.91 - $38.75 hr

Care Coordinator Trainee:  $23.94 - $32.08 hr

GENERAL STATEMENT OF DUTIES

Care Coordinator 

Provide care coordination and case management; facilitate team meetings: develop wraparound care plans including professional and natural supports to meet the intensive community-based treatment and support services needs of individuals and their families; provide direct services to individuals and their families by facilitating strengths and needs assessments and referrals to appropriate agencies or services; support the development of a system of care and assist in service coordination with partner agencies.

Care Coordinator Trainee

Under close supervision, provides care coordination and case management; facilitates team meetings; develops wraparound care plans including professional and natural supports to meet the intensive community-based treatment and support services needs of individuals and their families; provides direct services to individuals and their families by facilitating strengths and needs assessments and referrals to appropriate agencies or services; supports the development of a system of care and assists in service coordination with partner agencies.

Work under the general supervision of a supervisor who assigns work, establishes goals and reviews the results obtained for overall effectiveness.

DISTINGUISHING CHARACTERISTICS

Care Coordinator Trainee is the entry-level classification within the Care Coordinator series and is distinguished from the journey-level Care Coordinator by level of responsibility assumed and the complexity of the duties performed. The Care Coordinator classification has a maximum case load of fifteen and may be complex in nature.  Care Coordinator Trainee employees participate in a nine-month comprehensive credentialing process during which caseload complexity and quantity are significantly less than the journey-level classification and rigorously monitored.

  • Orient individuals, families and teams to program values and process.

  • Develop strength/needs assessment with input from individual, family and involved professional and natural supports.

  • Complete a functional behavioral assessment and develop a crisis/safety plan.

  • Develop a wraparound plan of care addressing the strengths and needs for individuals and families across multiple domains; facilitate team meetings with a consensus-based collaborative decision-making process.

  • Facilitate the collaborative development, periodic review and revision of the wraparound plan of care including a proactive crisis and safety plan.

  • Ensure shared accountability for all team members and is responsible for the development of outcome-based goals and measurement strategies.

  • Participate in intra-agency and interagency case staffing meetings for purposes of evaluation and treatment planning for individuals.

  • Serve as an advocate/liaison in assisting individuals to obtain necessary services.

  • Ensure that the team meetings and all supporting mental health services are culturally and linguistically responsive, person-centered, and emphasizes individual and family voice and choice in the decision-making process.

  • Adhere to system of care values, principles of wraparound, and support a strength-based approach at all levels.

  • Support the development of a system of care and assist in service coordination with partner agencies.

  • Maintain practice and documentation in accordance with health department, high-fidelity programs and OARs.

  • Provide high quality wraparound services in accordance with the model. Participate in all fidelity reviews, coaching and meeting observations.

  • Perform other duties as assigned.

Response Level 2: In the event of an emergency, employee makes a reasonable effort to report to work. Employee may be required to perform duties outside of their regularly assigned duties. Employee may be requested to work an extended or flexible schedule. Staff at this response level must complete Incident Command System 100 and 700 on-line or in-person training.EXPERIENCE AND TRAINING

  • Three years of relevant work experience in a behavioral health services field: OR

  • Bachelor's degree in a behavioral science field; OR

  • Any satisfactory combination of work, education, training, or experience relevant to the position, as determined by Marion County.

SPECIAL REQUIREMENTS

  • This position provides services to monolingual Spanish-speaking individuals; therefore, fluent bilingual Spanish/English skills are required. An additional 5% will be added to the hourly wage as compensation for this skill. Finalist will be required to pass a verbal and written Standardized Spanish/English Fluency Assessment as part of the hiring process.

  • This position may be subject to the following:  Must not be excluded from participation in the federal health care programs (Medicaid, Medicare and other federally funded programs that provide health benefits); AND must not be excluded from participating in federal procurement (Federal Acquisition Regulation) and non-procurement activities (Executive Order No. 12549).

  • Must possess a current driver's license in the applicant's state of residence and an acceptable driving history. Marion County will obtain a copy of the driving record for all qualified applicants from Driver and Motor Vehicle Services and review the driving record according to the Marion County policy and procedure for Driving on County Business.   • The finalist for this position will be required to pass a criminal history background check, including finger printing; however, conviction of a crime may not necessarily disqualify an individual for this position.

  • This is a safety sensitive/special needs position. Final candidates selected for this position will be required to pass a pre-employment drug screen, including testing for marijuana.   • This is a full-time position, which is eligible for overtime.

  • Typical Work Schedule: Monday

  • Friday, 8:00 a.m.

  • 5:00 p.m., with flexibility depending upon the needs of the department and program.

KNOWLEDGE, SKILLS, AND ABILITIES

Working knowledge of the general concepts of mental health and chemical dependency as well as community resources; ability to establish positive working relationships with employees, individuals and their families, other agencies and the public; ability to prepare accurate reports and to document individual services in a timely and professional manner; ability to work within a team and collaborate with team members; knowledge of and ability to act within professional code of ethics and to maintain appropriate confidentiality.

PHYSICAL REQUIREMENTS

Operates a motor vehicle; sees using depth perception; stands; sits; moves about the work area; bends forward; operates a keyboard; speaks clearly and audibly; reads a 12-pt. font; hears a normal (or whispered) level of speech; works in areas that may be exposed to humidity.We offer the following retention bonus, prorated based on FTE:

  • $2,000 bonus after 6 months of continuous employment
  • $2,000 bonus after 12 months of continuous employment

  • Up to $20,000 to reimburse for tuition, licensure and/or other educational expenditures. According to the Marion County policy administrative policies regarding tuition payments.
