JOB SUMMARY: The Bereavement Coordinator is responsible in planning, implementing and maintaining a bereavement program to meet the needs of families/caregivers from Bristol Hospice for up to one (1) year following the death of the individual hospice patient.

ESSENTIAL JOB FUNCTIONS:

  • Coordinate the assessment and delivery of grief counseling needs and services (one-on-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers

  • Plan for Bereavement Program development, expansion and refinement through annual evaluation of services

  • Provide bereavement support to hospice personnel coping with work related grief through one-on-one counseling, supervision of interns providing one-on-one counseling, and referral to community resources

  • Provide bereavement information and referral services to callers from the community

  • Expand and maintain community resource list, as needed

  • Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups)

  • Function independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community

  • Participate in hospice activities such as in-service education, hospice personnel meetings and relevant committees

  • Advance professional knowledge and skills

  • Actively participate in quality assessment performance improvement teams and activities

  • Accept and perform other related duties and responsibilities, as required

Qualifications

REQUIRED EDUCATION & EXPERIENCE:

  • Must possess a bachelors degree or equivalent theological degree from a college or university OR Master's degree in Social Work with qualifications of experienced clinical grief counseling

  • Must have experience with families/caregivers and patients through death/dying issues

REQUIRED KNOWLEDGE, SKILLS & ABILITIES:

  • Must possess the ability to work as a team member

  • Must possess the knowledge and commitment to hospice philosophy

  • Must demonstrate good verbal and written communication, and organization skills

  • Must possess an understanding of grief response and experience with counseling individuals and/or in groups regarding loss

OTHER REQUIREMENTS:

  • Must be able to travel often without notice

  • Must be flexible in work hours

WORK ENVIRONMENT:

  • Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations

  • Must comply with harassment and discrimination policies

  • Must foster good working relationships

  • Must be dependable and flexible in work schedule

  • Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order

  • Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter

PHYSICAL ENVIRONMENT:

  • May have to travel in inclement weather

  • May be required to stand and sit for long periods

  • Must be able to hear and see with or without aid

  • Must be able to read and follow instructions as required
