Description
Developmental Pathways (DP) has an opportunity for a Benefits Support Coordinator in our Systems Navigation department to serve as a subject matter expert on Medicaid and Social Security eligibility determination and applicable programs and services; Serves as a resource/point of contact for complex cases. The Benefits Support Coordinator will assist individuals, families, and case management staff by providing various levels of Medicaid/benefits support. The Benefits Support Coordinator will ensure timely completion and timely submission for approval of funding-related documents.
This is a 40-hour week, hybrid position, working primarily remotely and in-person, coming into our headquarters office in Aurora, CO, and/or other offsite locations as needed.

Requirements
Bachelor's Degree; or
Five (5) years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to the requirements of the position
Personal vehicle in good operating condition for use during work as appropriate
Valid Colorado motor vehicle license and proof of motor vehicle insurance
Ability to meet our driving requirements - no suspensions/ no more than two tickets in the past three years
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.

Additional Responsibilities
Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases
Complete various assessments to determine eligibility for services/supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools
Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered
Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members
Maintain knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs, and other community resources
Maintain a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
