Job Duties
Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a 94-bed medical facility providing acute medical diagnoses, skilled care, intermediate care, and long-term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities.
Admissions to HWDMC are generally either permanent placement or special hospitalization. Patients typically come to HWDMC as a special hospitalization for step-down to stabilize, post-surgery rehabilitation, or long-term care.
The Social Work Assistant
Minimum Qualifications
~ Knowledgeable of human behavior and development, therapeutic counseling, social systems and social work principles and practice
~ Knowledge of mental health diagnoses, behaviors and intellectual disability.
~ Knowledge and ability to provide social work case management and coordinate resources.
~ Ability to individually develop goals, plans and interventions according to patient needs.
Additional Considerations
~ Working knowledge of applying for Medicaid, Medicare, and Social Security Benefits is highly preferred.
~ Skilled in gathering the information needed for applications and understanding the process of applying for benefits.
~ Ability to provide social work services to patients;
~ Coordinate agency and community resources;
~ Communicate orally and in writing all assessments and interventions with patients, families and external agencies; and to work effectively to solve problems.
~ Knowledge of mental health diagnoses, behaviors and intellectual disability.
~ Knowledge and ability to provide social work case management and coordinate resources.
~ Ability to individually develop goals, plans and interventions according to patient needs.
~ Knowledgeable of NASW Code of Ethics.
~ Ability to multi-task with flexibility.
~ Ability to participate on an Interdisciplinary Team and be a contributing member.
