Position Purpose: Perform duties related to the day-to-day operations of the Service Management functions for Foster Care members to achieve the highest possible levels of functioning and wellness by ensuring consistent, timely access to medically necessary behavioral health services. Monitor quality of care, assist with discharge planning.

Work with Foster Care Members, Medical Consenters, Department of Family and Protective Services (DFPS) staff and providers to meet the behavioral healthcare needs of each foster care member's condition.

Identify foster care members who may benefit from service management, conduct an assessment and develop, implement and monitor a healthcare service plan.

Provide information to DFPS staff to facilitate development of the DFPS case plan.

Update database regularly with most recent Service Management Plan of Care.

Review information submitted by providers into the database to ensure material is appropriate and consistent with guidelines for data inclusion.

Encourage behavioral health providers to use evidence-based practices and confirm that behavioral health providers and PCP's are sharing information as required.

Comply with established referral, pre-certification and authorization policies, procedures and processes by related medical management staff.

Maintain compliance with federal and state regulations and contractual agreements.

Coordinate service management functions with other departmental functions.

Monitor data to address trends or potential quality improvement opportunities.

Maintain HIPAA compliance.

  • Full license required- RN, LCSW, LPC or LMFT

  • This position will be seeing members in rural areas. Travel could be up to 150 miles

  • Candidates from the following counties or areas are highly preferred- Midland/Odessa, Andrews, Pecas, VanHorn, or El Paso

Education/Experience: Master's degree in Behavioral Health. 3 years of case and/or utilization management experience.

Experience in psychiatric health care settings, child behavioral health conditions and members with special needs. Working knowledge of utilization review procedures, and familiarity with mental health community resources. Thorough knowledge of a specialized or technical field such as clinical nursing, case and/or utilization management involving knowledge plus the application of basic theory.

Previous experience as a lead in a functional area, managing cross functional teams on large scale projects or supervisory experience including hiring, training, assigning work and managing the performance of staff. Foster care experience strongly preferred.

License/Certification: Unrestricted license as a LCSW, LMFT, LMHC, LPC, PhD, PsyD or RN.

VIEW JOBS 12/19/2018 12:00:00 AM 2019-03-19T00:00 This position will focus on mental health care coordination and support for identified patients with complex behavioral health needs. Working under minimal supervision, this position work with patients and their families to maximize the psychosocial functioning and adjustment to living with ESRD and/or chronic kidney disease.. The Behavioral Health Specialist must also be able to work successfully both in person and remotely, via telephone or videoconferencing technologies, with a multi-disciplinary team comprised of physicians, nurse practitioners, nurses, care coordinators, clinic social workers and other nonclinical teammates. In this role you will: * •Collaborate with multidisciplinary treatment team including patients' primary care providers, care coordinators, quality team, and others. * Arrange, coordinate, monitor, evaluate, and advocate for support services to meet patient behavioral health needs. * Ongoing monitoring and revision of care coordination and/or connections to outside agencies as needed. * Educate patient and patient family (when appropriate) on condition, treatment, and benefits of selected intervention. * Inform patient and caregivers of relevant support services as appropriate. * •Support and coach dialysis nurses on effective identification of emotional and psychosocial health needs, and promote sensitivity to mental health in their daily practice. * Use Motivational Interviewing, Cognitive Behavioral Therapy, Mindfulness Techniques, Family Therapy, & other therapeutic techniques to assist with care coordination. * Conduct bio-psychosocial and mental health assessments; create individualized support plans, and review and complete patient record documentation. * •Clinical focus on depression, anxiety& other behavioral health and adjustment issues with short, solution-focused approach. * •Short-term interventions with additional intervention time spent with difficult to engage patients. * •Complies with all state and federal mandatory reporting laws such as Adult Protective Services and Child Protective Services. * •Adheres to all confidentiality and HIPAA standards as required by law. * •Other duties as assigned Qualifications for our Shinning Star: * Required Clinical Social Work Licensure (i.e. LCSW/LICSW, or state equivalent) * 3-5 years experience preferably experience in Behavioral Health * Master's degree in Social Work (MSW) with Clinical Concentration from a CSWE accredited program * Experience working with government and/or commercial insurance programs a plus * Experience working with kidney care patients or other chronically ill patient populations, grief and loss, and/or in a hospice setting, highly recommended. * Demonstrated experience in mental health support modalities such as individual therapy, cognitive behavioral therapy, dialectical behavioral therapy, solution-focused therapy, motivational interviewing, group therapy, and crisis intervention. Knowledge of the following: Principles and practices of clinical or rehabilitation counseling; Familiarity with social determinants of health and impact on mental health disorders;Community resources and assistance agencies; Group dynamics * Comfortable working with patients remotely, including by telephone and/or videoconference. * Demonstrated ability to establish and maintain effective working relationships with patients, staff, and the public. * Ability to travel within assigned market, rounding on patient census * Intermediate level proficiency in MS Outlook (other email programs), Word, Excel and PowerPoint * Bilingual in Spanish/English Highly preferred.
