Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital located in Torrington, Connecticut, that serves as a regional health care resource for the 100,000 residents of Litchfield County and Northwest Connecticut. CHH offers personalized attention from an expert team of caregivers and physicians that utilize advanced technology and clinical partnerships in a convenient, safe and comfortable patient environment.

Will provide administrative support to the Medical, Clinical, and Administrative staff of Outpatient Behavioral Health. This will include performing the critical task of greeting and interfacing with behavioral health patients and community partners.

Check-in all patients attending services. Complete required registration, charging, and payment collection for each patient.

Assure that patient demographic and insurance information is current. Work with patients to complete and sign required forms, permissions, pre-appointment scales, and surveys as assigned.

  • Associates degree in related field with one or more year's job related experience.

  • Excellent communication and patient interaction skills

  • Strong computer skills

  • Exceptional organizational skills to support the efficient operation of the department

Child First (Child and Family Interagency Resource, Support, and Training) is an evidence-based in home early childhood intervention, embedded in a system of care that works to decrease the incidence of serious emotional disturbance, development and learning problems, and abuse and neglect among the most vulnerable young children and families. The Data Specialist will be responsible for supporting continuous quality improvement, ongoing monitoring and audits of data to ensure fidelity to Child First model, Child First benchmarks and Accreditation standards. Support outreach efforts and provide limited administrative supports to clinical coordinator. Helps Clinical Coordinator to plan, design, implement, and maintain a comprehensive continuous quality improvement (CQI) program to ensure fidelity to the Child First model. Assists in educating staff about QI systems and requirements. Meets with Clinical Coordinator and internal audiences to identify and problem solve QI issues. Monitors program efforts to ensure compliance with Child First benchmarks, accreditation standard measures, client satisfaction surveys, and reports on results. Performs audits of electronic medical records (CFCR) and reviews other data documents/systems (ASD, etc.) to ensure complete and accurate documentation. Helps to prepare various monthly and quarterly reports. Creates and manages program outreach materials, may include community presentation materials. Records and maintain group supervision meeting notes including agenda, discussions, decisions, and follow ups. Attends meetings and engage in trainings as needed to perform duties and support program needs. Performs other duties as assigned or as necessity dictates. Qualifications * Bachelor's degree * Experience with timely and accurate data collection and entry and providing outcome reports. * Experience working in early childhood system of care within North West Region of CT. * Knowledge and/or experience with providing in home services for young children and their families. * Knowledge and experience working diverse cultures and ethnicities * Knowledge of continuous improvement principles * Ability to understand and use analytical reasoning to interpret data * Excellent oral and written communication skills * Exceptional listening and analytical skill * Highly organized, self-motivated, and flexible, * Strong computer skills especially in Word and Excel * Strong commitment to the vision, mission, and goals of Child First.
