The Behavioral Health Navigator (BHN) works within the Community Health and Behavioral Services (CHBS) program and is responsible for assessing Social Determinants of Health (SDOH) needs of individuals presenting to CHBS.
Core Responsibilities
Conduct and schedule assessments and evaluate needs for adults and children.
Address needs and barriers to treatment identified in the assessment process.
Address SDOH needs to promote the best possible outcomes for individuals served.
Provide crisis support, in collaboration with the CHBS clinical team and Crisis Triage Workers.
Provide rapid intensive intervention to homeless individuals with a suspected or documented mental illness/substance use problem.
Support individuals with the development of coping skills and identification of resources to reduce future stressors.
Develop and maintain an effective communication network with clients, staff, community agencies, and other agency departments.
Provide information and referrals and facilitates linkage or individuals to appropriate services.
Provide advocacy, counseling and support to individuals as needed.
Complete all required documentation according to program regulations.
Participate in agency or community activities as part of the treatment team.
Engage in outreach efforts to promote awareness of Community Health and Behavioral Services.
Attend appointments with individual to ensure engagement with service.
Qualifications
Associates degree in Human Services, Sociology, Psychology, LPN or related field.
1-2 years of case management related experience preferred.
Must have a valid Driver's License.
