Summary:

The Behavioral Health Counselor is a member of the UCHealth Primary Care team and works in collaboration with other primary care providers to manage the overall health of their patients. The Behavioral Health counselor will assist the primary care providers in recognizing, treating and providing focused intervention for patients in need of behavioral health services. The Behavioral Health Counselor will provide warm interdisciplinary handoffs by identifying, referring and advocating for patients who could benefit from specialty behavioral health services including telepsychiatry.

FTE: 1.000000 (FTE x 80 = hours per pay period)

Responsibilities:

Behavioral Health Counselors will provide direct clinical service in a primary care setting.

Patients will be assessed and treated for mental health and substance use issues in the primary care setting with support from a psychiatrist or psychiatric nurse practitioner via telepsychiatry.

Patients will have access to specialty psychiatric assessments and services through expedited referral to outpatient practices at UCHealth or CU Department of Psychiatry.

Embedded clinicians will have professional development opportunities through faculty in the CU Departments of Psychiatry and Family Medicine

Requirements:

  • Minimum Required Education: Master's Degree in Social Work, Counseling, Psychology, or a related field.

  • Required Licensure/Certification: State licensure as Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Addiction Counselor (ACD).

  • Minimum of two years' experience as a therapist preferred.

Behavioral Health Clinical Navigator I In this (special at-will) grant funded position, you will play a critical role in the Community and Public Health (CPH) division of the Colorado Springs Fire Department (CSFD). You will be responsible for performing assessments, intensive case management, behavior modification, and solution focused treatment for members of our Community Assistance Referral & Education Services (CARES) programs. Essential Job Functions – the basic job duties an employee must be able to perform: * Conduct intensive case management and community navigation for members on a daily basis; manage a caseload of high-needs clients * Complete behavioral health intake/assessment for those individuals referred to the CARES program * Conduct intakes/interventions in homes and other semi-public locations * Coordinate care for CARES members with primary care, behavioral health, and community partners * Implement behavioral modification/solution focused treatment for issues that are hindering improvement * Conduct psychoeducation specific to the client's reported diagnoses and symptomology * Create intervention planning/goal setting for each member assigned * Participate in case consultation, internal and external, to assist compliance of member * Interact in professional-to-professional communication to advocate for member * Complete case conceptualization/differential diagnosis/presentation in case review, internal staffings, and community meetings * Represent CPH at community meetings and report outcomes to supervisor * Assist with referral management, input, and acquisition * Provide community training and education * Assist with community outreach, building relationships with partners/agencies, and maintaining positive alliance * Monitor emergency medical response interactions and notifications for compliance We are looking for candidates who demonstrate: Knowledge of: * Appropriate scope of practice expectations * The DSM-V and Cognitive Behavioral Therapy/Solution focused treatment/ Systems model * High needs, severely mentally ill clientele * Substance abuse and polypharmacy concerns/treatments * Goal setting and treatment planning Ability to: * Complete assigned tasks with minimal supervision * Demonstrate leadership and task management skills * Formulate cohesive intervention plans * Recognize when to seek additional supervision/consultation * Identify any ethical concerns that might arise and discuss with supervisor * Represent CSFD in a professional capacity * Accept constructive criticism and demonstrate a willingness to learn * Demonstrate technical writing skills * Decipher research materials/outcomes * Navigate electronic medical records independently after training * Set clear and firm boundaries with clients * Function in a complex and fluid work environment * Communicate professionally, effectively, and efficiently with all stakeholders?
