Our Integrated Behavioral Health Program continues to grow! With one of the nation's leading integrated behavioral health programs following the PCBH model, Community Care Physicians is excited to be announcing a number of opportunities to join our growing team. These opportunities are available for full and part time positions creating options for flexibility to best promote self-care and work/ life balance. Opportunities at CCP are diverse and include working closely in a team-based setting, side by side with physicians and other clinicians on the patients care team in delivering evidence-based behavioral health interventions and treatment.

Mental Health Professionals Who:

Responsibilities:

  • Assessment, screening, consultation/ counseling

  • Brief intervention for psychological/psychiatric and behavioral problems/ disorders delivering solution-focused treatment, improving patient care and outcomes.

  • Behavioral intervention for disease management and lifestyle medicine.

  • Documentation, treatment planning, and consultation with patient's physicians and care team.

  • Providing orientation, consultation, and ongoing training to pertinent medical care steam members as needed in order to strengthen the relationship between primary care and behavioral health services and ensure a team-based model of health care delivery.

Annual salary range: $70,000 - $96,000

  • please note, this range is based on a 40 hour work week.

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
