Purpose of Job: Improve the overall health of the communities we serve by providing support of clinical programs and patient care as follows:

Essential Duties and Responsibilities:

  • Provides project assignment and guidance to BHCN staff.

  • Supervises, supports, trains and coaches those within team to improve their professional knowledge and skills.

  • Participates as directed in the screening of new patient requests to ensure that all individuals are adequately and appropriately served according to their individual needs.

  • Responsible for fostering teamwork, positive employee engagement and productivity, meeting and exceeding goals, objectives and metrics while ensuring excellent customer service to internal and external customers and patients.

  • Partners with department leadership to develop goals, workflows, and improvement processes.

  • Supervises Behavioral Health Care Navigators on-boarding and ongoing training.

  • Develops and regularly reviews appropriate service plans with patients and direct reports to implement appropriate services and developing effective alliance with patients to achieve more effective personal, social, educational, and vocational development.

  • Provides on-going supportive and/or case management functions to include group and individual appointments in accordance with the problems, needs, and strategies identified within the service plan to help the patients to achieve the stated goals and objectives.

  • Works collaboratively with patients to access CHAS Health services, and outside entities, facilitate effective transitions of care for hospitalized patients, arrange housing, transportation and address other social determinants of health.

  • Maintains and supports team with close communication with Behavioral Health Provider for input on program compliance, progress on goals and overall engagement or lack of in services.

  • Provides or facilitates at-home or community-based visits with patients and staff as needed. · Provides clinical supervision as assigned. · Targeted Care Coordinators Only: o Coordinates and facilitates the Child and Family Team (CFT) Interdisciplinary Team Meetings face-to-face with the family and member present. o Works with the CFT to develop an outcomes-focused, strengths-based person-centered service plan that includes both formal and informal services and supports. Idaho Locations: Targeted Care Coordination must be consistent with the Principles of Care and the Practice Model of the Idaho Youth Empowerment Services (YES) system of care. Works with the member's clinician to update the CANS at least every 90 days or more frequently as needed. If the Targeted Care Coordinator is certified in CANS and has access to the ICANS platform, they may complete the CANS initial/annual and updates. · Completes a variety of assignments simultaneously, with ability to work according to deadlines. · Performs other duties assigned, including supporting the CHAS Health Mission and Core Values. Qualifications: Bachelor's Degree in Human Services field required. 3 years of experience in a Behavioral Health setting required. Washington Locations: Agency Affiliated Counselors Registration issued by the Department of Health required within 90 days of hire. Idaho Locations: Certified Case Manager (CCM) through the Commission for Case Manager Certification preferred. 2 years of experience as a supervisor preferred. Effective oral and written communication skills required. Detail-oriented, organization, leadership, and self-motivation required. Must be able to work well in team environment. BLS (CPR & AED) required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required. Driver license required. Computer skills required.

Physical Demands: Must be able to move around the facility between 1/3 and 2/3 of the day, as well as sit at a workstation or in patient rooms. Using hands occurs over 2/3 of the day, while reaching with arms occurs less than 1/3 of the day. Communicating is also required over 2/3 of the day, while climbing, stopping or crawling is minimal. Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. occurs only occasionally.
