Due to our ongoing business relationship with the County of San Diego, Optum Behavioral Solutions has an immediate need for a Care Advocate. As the successful candidate you will focus on inpatient authorizations for psychiatric and chemical dependency patients.

  • This position requires working a Monday-Friday, 10:30am-7:00pm daily schedule*

Primary Responsibilities:

  • Making patient assessments and determining appropriate levels of care

  • Obtaining information from providers on outpatient requests for treatment

  • Determining if additional clinical treatment sessions are needed

  • Managing inpatient and outpatient mental health cases throughout the entire treatment plan

  • Administering benefits and reviewing treatment plans

  • Coordinating benefits and transitions between various areas of care

  • Identifying ways to add value to treatment plans and consulting with facility staff or outpatient care providers on those ideas

Required Qualifications:

  • Licensed Master's degree clinician in Psychology, Social Work, Counseling or Marriage and Family Counseling, or a Licensed Ph.D. / PsyD; or an RN with 2 years behavioral experience

  • Licenses must be active and unrestricted in the State of California

  • 2 years of experience in a related mental health environment

  • Intermediate level of computer proficiency specifically with MS Word and Excel

  • Must be able to obtain and maintain access to the County of San Diego's electronic health record, Anasazi

Preferred Qualifications:

  • Experience in a managed care setting

  • Dual diagnosis experience with mental health and substance abuse

  • Knowledge of the Medi-Cal system

  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients

  • Previous experience in a telephonic role

  • Strong data entry skills

The job The Bilingual Home Advantage Care Navigator enhances seniors' ability to manage their health and to live safely and independently in their home by supporting the Home Advantage Occupational Therapist (OT) in developing and implementing home safety plans. You will * Receive completed Occupational Therapist Home Safety Evaluation report and drafts the member's Home Safety Plan that includes the OT recommendations. * Conduct home visit following OT visit in order to finalize the Home Safety Plan with the member. The Safety Plan is designed to help supports member safety, prevent falls and injury. Helps member to understand how to implement the OT recommendations, utilizing coaching/education, member's support system, and community resources. Refers to care management as appropriate. * Provide ongoing telephonic communication as needed and requested by member to support the safety plan, member's understanding and use of benefits, assistance in scheduling appointments, development of a support system, and reinforcement of self-management. * Assist members and caregivers in taking an active role in their health, safety and independence through coaching, education, navigation and referrals to appropriate care and community based resources. * Complete timely and accurate documentation to record home visit, telephonic communication with members, and all other activities. * Utilize department desktop procedures, workflows, job aids and training materials. * Maintain the member's right to privacy and protects SCAN by keeping information confidential. * Adhere to all quality, compliance and regulatory standards to achieve desired outcomes. * Actively support the achievement of SCAN Vision and Goals. Requirements Your qualifications * Associate's Degree in Social Work, Human Services or a related field or equivalent work experience * Strong interpersonal skills, including excellent written and verbal communication skills. * Strong organizational skills. * Ability to multitask * Must be bilingual in English and Spanish: conversation, reading and writing. * Proficient in Microsoft Outlook, Word, Excel and PowerPoint * Valid California Driver's license.
