Position Purpose: Perform duties related to the day to day operations of the Integrated Case Management functions to include working with members identified as high risk to identify needs and goals to achieve empowerment and improved quality of life for both behavioral and physical health issues.

Assess members' current functional level and, in collaboration with the member, develop and monitor the Case Management Care Plan, monitor quality of care; assisting with discharge planning, participating in special clinical projects and communicate with departmental and plan administrative staff to facilitate daily operations of the Integrated Case Management functions. Collaborate with both medical and behavioral providers to ensure optimal care for members.

Work telephonically with patients identified as high risk, for both behavioral and physical health issues, and their providers to identify needs, set goals and implement action steps towards achieving goals. Empower patients to help them improve their quality of life and ensure an integrated approach to address complex issues.

Understand and comply with NCQA guidelines and HEDIS measures.

Comply with established referral, pre-certification and authorization policies, procedures and processes by related Medical Management staff.

Education/Experience: Master's degree in behavioral health and an unrestricted license as a LCSW, LMFT or LPC, or a PhD, PsyD or RN. 3 years of case and/or utilization management experience.

Experience in psychiatric and medical health care settings. Working knowledge of mental health community resources.

License/Certification: Unrestricted license as a LCSW, LMHC, LMSW LMFT, LPC, PhD, PsyD or RN license in applicable state.

Minimum Education and Experience Requirements Requires a Master's degree from an accredited college or university with major coursework in social work, gerontology, psychology, counseling or closely related field to area of assignment. Dependingupon area of assignment, six (6) months of clinical case management may also be required. Special Requirements Depending upon area of assignment, may be required to work nights/weekends/holidays. Management may assign additional functions related to the type of work of the job as necessary. Conducts clinical screenings, interviews and bio-psychosocial evaluations in hospital district. Coordinates treatment with community, governmental and service providers. Conducts home and community visits as needed to provide substance use disorder counseling to participants and their families. Observes, evaluates, and monitors reports and records program participant status and progress. Conducts clinical interviews and biopsychosocial evaluations. Conducts group and individual counseling sessions. Develops treatment plans with program participants and/or their families. Maintains associated clinical records manually and/or electronically using computer based applications. Prepares accurate and timely written reports on assigned caseload using computer based software applications. Observes, evaluates, monitors, reports and records client status and progress. Counsels program participants, including those with complex issues; facilitates intimidate and advanced group sessions concerning substance abuse and co-occurring related issues. Provides advocacy services to clients (greets family, obtains consent for medical exam, walks client to the exam room, provides emotional support during the examination, provides counseling service referrals and any other referrals family may need). Answers after hours calls from front desk, and from the 24-hour rape hotline. Assists with physical abuse intakes for Crisis Protection Team. Acts as an advocate between the families and law enforcement, state attorney's office, or child protective services to ensure that the needs and/or concerns of the client are being addressed. Links families to different agencies within the community, such as housing assistance, childcare, and food banks, to assist them with various needs. Assists clients with completing assistance applications. Participates in staff meetings and trainings as scheduled. Coordinates treatment with other community, governmental and service providers. Prepares accurate and timely written reports on assigned caseload as well as the quality and/or quantity of work completed. Confers with program participants, their families and/or other care-givers regarding issues of treatment. Documents progress notes and tracks daily reports for documentation purposes. Assists with special projects assigned by the supervisor. Performs related work as assigned. WORK ENVIRONMENT Physical Demands Physical demands refer to the requirements for physical exertion and coordination of limb and body movement. Performs sedentary work that involves walking or standing some of the time or sustained keyboard operations.
