Job Description
The Behavioral & Mental Health Therapist I will participate in a model of care centered around integrated, multidisciplinary services, with seamless collaborations with psychiatry, psychology, social work, medical specialties, and other behavioral and mental health disciplines.
The Supportive Care Clinic is expanding and actively seeking a licensed clinical social worker or licensed professional counselor for the role of Behavioral Mental Health (BMH) therapist as well as to act as the interdisciplinary psychosocial clinician in Supportive Care Clinic, an outpatient palliative oncology clinic for children with cancer and their families who sustain significant challenges to their quality of life. The BMH therapist will provide evidence based individual and group therapy focused on a broad range of conditions, including anxiety, mood disorders, physical and somatic conditions. In addition, the therapist will conduct interdisciplinary visits with other members of the Supportive Care team (MD, NP, psychologist). All patients are referred internally from Aflac Cancer and Blood Disorders Center providers, and all have a life-threatening or life-limiting illness. Appointments will be a mixture of in-person and telehealth. The focus is on physical, emotional, social, and existential suffering, family coping, sibling challenges, anticipatory grief, end-of-life, hospice coordination, and bereavement. The Behavioral & Mental Health Therapist I will work closely with Supportive Care psychologist, other Aflac psychologists and social workers, the outpatient and inpatient interdisciplinary palliative care team, psychiatry, chaplains and family support team members to develop and implement a comprehensive treatment and care coordination plan for each patient.
Experience
Preferred Qualifications
Experience with Unified protocol, CBT, ACT, DBT, TF-CBT, PCIT, and/or Solution-Focused Therapy
Experience with children & adolescents with co-occurring medical issues

Job Responsibilities
Patient Care
Provide evidence-based and effective delivery of services (e.g., assessments, consultation, and therapy) appropriate to the patient's specific needs and severity level of disability or impairment, and make referrals as needed.
Gather and integrate information from all available sources, including client self-reports, reports from family members, guardian(s), documented medical records, treatment history, etc.
Conduct crisis and safety planning with patients and coordinate transfers to higher levels of care as needed.
Maintain a therapeutic relationship with patient and family.
Respect and incorporate aspects of patient's and family's identity in care.
Include family/caregiver participation in all phases of the patient's program while communicating at the patient/family level of understanding.

Communicate with support and operational staff effectively to ensure patient continuity of care.
Assess client satisfaction and participate in program evaluation as needed.
Generate flexible solutions to potential barriers in patient care to benefit patients.
Administration
Complete notes in electronic medical record within time guidelines, including diagnostic coding, screenings, and treatment plans.
Bill appropriately for services in accordance with regulations set forth by the Centers for Medicare and Medicaid Services.
Participate in team meetings and case consultations (as needed).
Seek additional evidence-based training associated with role.
