Responsibilities

Care managers will assist person receiving outpatient services in adhering to treatment, encouraging responsibility for their treatment, maintaining follow-up appointments, scheduling and keeping appointments with other community medical providers, tracking of data and completion of agency documentation and problem solving with persons on potential barriers or issues that could impact recovery or progress. Services provided under the directive of supervising staff.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

1.Bachelors Degree in Behavioral Health Science from an accredited institution with 2 years post Bachelors experience in the provision of mental health services.

2.Valid Drivers License

VIEW JOBS Overview This Discharge Care Manager position is for the Wellness and Recovery Center of Warren County. The Discharge Care Manager provides care management assessment and support to adults diagnosed with a primary mental health or co-occurring diagnosis. The Discharge Care Manager assists in providing support to individuals who have stabilized and are awaiting linkage to the next level of care. This person in this position is knowledgeable of community resources and is an advocate for the clients. The Discharge Care Manager assists clinicians with referral/linkage to community resources as needed, and may provide transportation to assist clients as needed. The Discharge Care Manager works as part of a multi-disciplinary team and interfaces with both internal and external providers. Responsibilities The Discharge Care Manager participates as a member of The Wellness and Recovery Center team. This program provides behavioral health urgent care to adults in need of rapid support and intervention for an identified mental health concern. The goal of the program is to assist individuals in stabilizing acute symptoms through clinical support and medication management for a period of up to 30 days. The Discharge Care Manager will function as a member of this team and will assist with discharge planning, as well as addressing identified case management needs which support the individual's recovery. More specifically, the Discharge Care Manager will assist with expediting referrals to the next level of care for individual, link the person with community resources such as shelter, transport, or food pantries, and will provide ongoing supportive contact with individuals who have stabilized but are awaiting linkage. The Discharge Care Manager will use an agency vehicle to provide community outreach as needed/identified by the treatment team. Care is provided under the direction of supervising staff and the clinical team. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Bachelor's degree in a behavioral health science from an accredited institution. 2. 2 years' experience in a mental health setting strongly encouraged. 3. Valid Driver's License - as per policy requirement. 4.
