Overview

HOURLY RATE (DEPENDING ON SKILLS/EXPERIENCE): $28.89 - $31.78

SCOPE: The Bariatric Surgery Licensed Vocational Nurse is responsible for the coordination and delivery of services to bariatric patients under physician and practice manager supervision. Provides specialized care coordination including information gathering, patient education, training, and patient follow up.

STATUS: Part Time position - 8/10 hours/week.

Responsibilities

Workup Facilitation:

  • Provide patients with information on how, when, and where to schedule appointments for Bariatric workup.

  • Assist with signing up for the Bariatric Seminar and Support Group.

  • Communicate authorization status for all referrals.

  • Fax all external referrals with authorization to providers and confirm receipt. Notify the patient to schedule with the provider after receiving confirmation from the receiving office.

  • Schedule follow ups and other pertinent appointments for patients

Chart Preparation:

  • Request all consult notes completed and confirm upload to patient charts.

  • Request and confirm upload of all diagnostic exams done (i.e. labs, UGI, Chest X-Ray, etc.).

  • For patients with no weight check requirements, reschedule any pre-scheduled appointments if they have not completed the workup ordered.

  • Update appointments as needed for provider change (i.e. patient completes workup and needs pre-op with surgeon for consent)

  • Document and facilitate Epic Care's No Show Process/Documentation

Bariatric Apps: DoctorPlan and Baritastic

  • Sign patients up for Doctor Plan or Baritastic

  • Update the program every two weeks to capture any schedule changes. c.

  • Review information provided by patients prior to upcoming appointments and update charts as needed.

  • Participate in any application changes for Doctor Plan or Baritastic

  • Troubleshoot Doctor Plan or Baritastic issues for patients

Time Off Documentation:

  • Complete employer provided FMLA/Disability forms for all Bariatric patients. Copy to the chart and return to the patient by secure messaging such as My Chart, fax or by mail.

  • Coordinate pick-up of documents with in-office staff.

Pre-Op and Post-Op Calls

  • Call patients the day before surgery to confirm surgery itinerary, surgery instructions, and confirm all post-op medications have been received from the pharmacy. Also confirm post-op vitamins have been purchased by the patient to be started as directed by the surgeon.

  • Call patients 48 hours (two days) post-op for initial post-op check to monitor for any potential post-op complications and provide any additional patient education for initial two weeks post-op instructions. If patients are doing well, confirm 2-week post-op appointment with the surgeon. If the patient has any complications or needs additional support, schedule a follow up call with the nurse in 1 week to check in and update the surgeon as needed.

Additional Duties:

  • Communicate all pertinent information and any interventions to NP/Physician.

  • Train and support General Surgery LVNs and Medical Assistants in Bariatric Operations

  • Able to schedule patients and check patients out of telehealth clinics

  • Perform other responsibilities as assigned by Physicians and Practice Manager

Qualifications

MINIMUM QUALIFICATIONS:

  • High school graduate or equivalent.

  • Valid California LVN License required

  • IV/Venipuncture Certification required

  • BLS Certification required
