Overview

Responsibilities

Coordinates evaluation for patients referred for advanced cardiac therapies

Organizes and enters data submitted by referring physicians prior to patient's initial appointment.

Identifies gaps in information and takes appropriate action e.g. call to request further info., reschedule

the appointment) as observed by clinical staff.

Establishes working relationship with the transplant and VAD coordinators to address the efficient work

up and management of patients.

Obtains results of requested studies from referring healthcare providers.

Using the transplant and VAD protocols, schedule studies that must be done at Y-NHH.

Results must be received in a timely fashion and the program assistant must remain sensitive to the

personnel and agencies involved, as well as assure the information is correct.

Refers specific information to the transplant and VAD coordinators

Maintains Transplant Waiting List to ensure accuracy of patient information.

Identifies problematic issues and refers them to the transplant coordinator.

Responsible for organization of weekly team meeting.

Interfaces with other hospital departments/ personnel to ensure pertinent documentation has been

communicated to appropriate departments.

Works closely with the staff and the Histocompatibility Lab requiring documentation of blood types ,

date of blood samples for the lab and information for listing patients on the national registry.

Develops results oriented working relationship with staff members in other departments of the hospital ,

as well as agencies and companies outside YNHH.

Provides administrative support to the transplant and VAD coordinators as needed to ensure timely

communication of patient related information to both internal and external departments.

Coordinates the scheduling of diagnostic studies for transplant and VAD patients.

Qualifications

EDUCATION (number of years and type required to perform the position duties):

Associates Degree in Allied Healthcare or related field preferred.

EXPERIENCE (number of years and type required to meet an acceptable level of performance):

Two (2) years of related clinical experience or equivalent combination of education and experience ., Special Skills

Demonstrated, excellent organizational skills. Ability to handle multiple on-going tasks. Database management

skills preferred. Must be able to handle interactions with all levels of staff , patients and families.

Ensures accuracy of patient information. Maintains effective communication with transplant and VAD coordinator

team to optimize patient evaluation.

Responsible for coordinating clinical aspects of multi-organ transplantation as it applies to the transplant recipient. Assists in the development and growth of an individualized program for patients referred for possible transplantation. Responsibilities include coordination of the pre-transplant evaluation, registration and listing of potential transplant recipients, and post transplant care. As an integral member of an interdisciplinary team, coordinates and performs various other clinical, educational, administrative, and technical responsibilities related to clinical transplantation. . Clinical Practice * 1.1 Coordinates all recipient and potential donor aspects of transplantation including initial candidate referral, pre-transplant evaluation, candidate selection, wait listing, transplant admission, and post transplant follow-up care. * 2. Patient Satisfaction and Service Excellence * 2.1 Participates in the development of patient relations standards reflecting excellence consistent with hospital policy for internal/external customers in collaboration with YNHH and YNHTC leadership * 3. Performance Improvement * 3.1 Monitor clinical outcomes and system issues related to quality of care delivery to patients within YNHTC * 4. Organizational Leadership * 4.1 Interfaces with multiple hospital areas in coordinating patient education, psycho-social services and various other aspects of the transplant clinic experience. * 5. Key Interfaces * 5.1 Director of YNHTC Qualifications EDUCATION BSN /or appropriate certification/licensure. Masters Degree is also desirable.. EXPERIENCE Two (2) to five (5) years clinical patient care in an area relevant to transplantation. Actual experience in organ transplantation is highly desirable and preferred LICENSURE Registered Nurse or Physician's Assistant recommended Certified Clinical Transplant Coordinator (CCTC) recommended within 2 years of starting position SPECIAL SKILLS Highly developed clinical, communication, and interpersonal skills are essential.
