The Associate discharge planner is a member of the senior psychiatric unit located at Providence Milwaukie Hospital. Discharge planning is an integral part of the care of the 19 bed, inpatient psychiatric unit and consists of organizing, collaborating, and communicating with community placements, insurers, mental health agencies, housing agencies and the Oregon Health Authority to ensure a safe discharge. The associate discharge planner works closely with social workers on delegated tasks to coordinate the patient's care post-discharge to include letters, faxing, facilitating community assessment, transportation, vouchers, follow up appointments, and level of care determinations. An essential function is the organization of treatment team reviews and complex case conferences.

Required Qualifications:

  • Bachelor's Degree Mental Health/Social Services Or equivalent educ/experience

  • 4 years of Equivalent Experience when not having a bachelor's degree.

  • Coursework/Training Residential Treatment, Acute Care Inpatient or Behavioral Schools.

Preferred Qualifications:

  • 2 years Experience in an inpatient mental health setting.

  • 2 years

  • Adult Unit: Experience with adult population preferably in an inpatient setting.

  • Child and Adolescent Unit: Experience with child/adolescent population preferably in an inpatient setting.

  • Senior Unit: Experience with geriatric population preferably in an inpatient setting.
