THE FORUM is recruiting for a hospitality focused Assisted Living/Memory Care Manager to join our team! This role is responsible for the planning, coordinating, and directing of a wide and varied scope of activities and programming which meet the spiritual, emotional, intellectual, vocational, social, physical, and environmental needs of the residents.

Wage range: $ 79,000-115,000

OVERALL RESPONSIBILITIES:

The Assisted Living and Memory Care Manager is responsible for providing direct care and supervision of License Nurse, Lead Care Managers,(Med Techs) and Care Managers. Nurse Manager has the oversight of nursing care/services of all the shift including the accurate and timely administration of medications, perform treatments, procedures, proper documentation of all Assisted Living and Memory Care services; establish effective communication within and between work shifts; attending family conference meeting; always maintain and protect confidentiality of resident's information. Takes emergency calls, as needed. Ensures community adherence to all applicable federal, state, and local standards and regulations promoting regular compliance. Expertise in caring for those with dementia. Support and promote the eight dimensions of wellness, Heart Felt Connections, and ensuring that care is person centered and individualized. Promote and support the concept of a universal worker where all team members focus on engagement of residents during their entire shift in appropriate ways geared towards resident's cognitive abilities.

ESSENTIAL JOB FUNCTIONS:

  • Ensure delivery of all resident's services in Assisted Living and Memory Care

  • Deliver care that complies with state and local regulations.

  • Participate in the development of resident care services; assist with resident admissions; modify care services to reflect current health status of residents; properly utilize resident care services.

  • Supervise delivery of care on all shifts; Monitor work of all team members to ensure that all medications orders and treatments are performed and documented.

  • Provide direct resident care; responsible for documentation and charting; ensure proper documentation as required.

  • Responsible for the overall schedule for all shifts, payroll reports.

  • Ensure adequate shift staffing to maintain continuity of resident care; inform supervisor of scheduling or staffing problems.

  • Assist with coordination of Health Center services that involve firsthand care to residents, i.e., navigator, care services, food & beverage, therapies, activities, housekeeping etc.

  • Respond to all resident requests (call lights included) for assistance.

  • Responsible to use and learn Medical Records (EMR) to dispense medications, receive and follow up orders from physicians, and enter orders in EMR and also perform all other duties within PCC.

  • Respond to all medical reports including the Pharmacy reports, lab reports and keep medication carts clean and organized.

  • Completes Heart Felt Connections training

  • Completes annual dementia training

  • Develops working knowledge of iNTL, Life Bio, and other life enrichment programs as required.

  • Admit residents and prepare necessary assessments.

  • Discharge residents and prepare necessary papers for discharge.

  • Attending care conference.

  • Conduct daily routine rounds of the residents in, participating in initial and ongoing residents' assessments to help identify needs and changes in condition.

  • Support new hire training and weekly/monthly in-service training.

  • Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the residents' needs. Demonstrate good decision-making skills. Provide written evaluation and oral counseling of assigned.

  • Practice positive resident relations; respond to resident and family inquiries, concerns, or problems according to The Forum Standards.

  • Perform other essential duties as required.

  • Observe significant changes in resident condition, report to resident's physician, responsible party, and document immediately.

  • Communicate and cooperate with members of inter-disciplinary health care team to accomplish resident goals established in residents care services.

  • Always maintain and protect the confidentiality of resident information.

  • Responsible for monitoring and teaching Medical Records (EMR-PCC) to dispense medications, receive and follow up orders from physicians, and enter orders in EMR.

  • Responsible for developing expense and labor budgets and for operating within budgets

Infection Control & Safety

  • Implement and practice infection control policies and procedures; ensure compliance by staff with universal precautions and all other infection control guidelines; inform Director of Health Services of any real or potential infection control concerns.

  • Monitor all infectious disease processes, residents, and staff, occurring in Assisted Living.

  • Practice all safety and loss prevention procedures; monitors adherence to safety rules and regulations and takes remedial action when necessary.

  • Other duties as assigned.

QUALIFICATIONS

  • License Vocational Nurse (LVN) state of CA

  • Three (3) or more years of supervisory experience in Senior Living Industry. Experience in Assisted Living, Memory Care preferred.

  • Effective communication (written and verbal)

  • Computer literate

  • Knowledge in all areas of local Health Department, OSHA, and other regulatory agencies relevant to healthcare services.

  • CPR and AED training kept current per The Forum's policies
