At Sunrise, our Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.

RESPONSIBILITIES & QUALIFICATIONS

Responsibilities:

  • Build meaningful relationships with a specified number of seniors and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities
  • Notify management of changes in condition and recommend adjustments in the level of care and service

  • Assist residents in life skills and other life enriching activities as indicated on their individual profile

  • Blends a variety of multi-sensory experiences into the resident's day

  • Participates in the development of the Individualized Service Plans (ISP) and monthly updates

  • Responsible for their designated group of residents during the shift, knows where their residents are and physically checks on them throughout the shift.

  • Partners with community team to ensure community is in compliance with OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.

  • Responds to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards

  • Invite, Encourage, Assist (IEA) residents in life skills and other life enriching activities as indicated on ISP and demographic profile

  • Committed to serving our residents and guests through our Principles of Services

Qualifications:
- Dedication to and passion to serve seniors with excellent customer service skills

  • Positive attitude, the flexibility to perform various duties in service to the residents and the ability to work in a team environment are keys to success- High School diploma/GED accepted and may be required per state regulations- In states where appropriate, must maintain certifications- Must be at least 18 years of age- Previous experience working with seniors preferred- Ability to make choices, decisions and act in the resident's best interest- Possess written and verbal skills for effective communication and a level of understanding- Competent in organizational and time management skills- Demonstrate good judgment, problem solving and decision making skills

The Utilization Management LVN Case Manager is responsible for managing low and moderate risk Special Needs Population (SNP) and Dual Eligible (DE) members. SNP / CMC Care Manager will ensure that the SNP and DE receive an assessment. Ensures the individualized care plan is developed and presented timely to the IDCT. * This is an office-based position, located in La Palma, CA Primary Responsibilities: * Managers a case load of 400 - 600 low to moderate risk SNP and or CMC members * Coordinates and schedules all IDCT meetings * Ensures all SNP and DE members are presented at IDCT at least annually * Facilitates Primary Care Provider attendance * Ensures SNP and DE member and or caregiver participation * Oversees the low risk SNP and DE population and ensures the annual review of the care plan is completed for those members who did not have a change in health status * Works collaboratively with the patient, physician, family/significant other and providers of healthcare to implement plan of care to meet the individual needs * Works collaboratively with discharge planners at the affiliated hospitals and case management staff to identify changes in health status. * Monitors care, which is easily accessible with no access barriers as related to the patient's available benefits * Provides accurate and up - to - date information to practitioners regarding clinical practice guideline criteria and patient information * Creates individualized treatment plans that incorporate the HRA, community LTSS services Required Qualifications: * Associates Degree in Nursing (ASN) * Current and unrestricted RN OR LVN license for the state of California * Minimum of 2 years of experience in a managed care environment, preferably in an IPA or MSO. Other case management experience will be considered * Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment Preferred Qualifications: * Bachelor's degree in nursing (BSN) * Bilingual Spanish speaking (ability to read, write, speak) * Case Management Certification (CCM) * Knowledge of and experience with the utilization of UM criteria such as Milliman & Robertson, Medicare and Medicaid guidelines, health plan criteria, etc. * Experience as a case manager, experience with utilization management * Experience working with Medicare members Careers with Optum.
