Overview Provides clinical and administrative support to the Assertive Community Treatment Program (ACT), a 24/7 operation, including the appropriate utilization and management of staff to ensure the quality and quantity of client care. Also ensures treatment provided is consistent with an interdisciplinary team approach to the delivery of care and VNS Health Home Care goals and objectives.

Works under general supervision. Provides oversight of high risk situations or crisis intervention as needed.

Assists in the development of treatment plans and goals based on a diagnostic assessment of the presenting problems and historical information, the establishment of long-term goals, and the administration of follow-up procedures. Provides advocacy and assistance in obtaining entitlements and community services to clients, families, and other caregivers. Provides home visits and carries a caseload as specified by the Program's level of service commitment and the client's needs.

Maintains productivity sufficient to meet VNS Health Behavioral Health goals. Assumes the Program Manager's responsibilities in his/her absence. Assists or provides direct support and evaluation of ACT staff as deemed appropriate.

Provides direct supervision to staff as needed. Organizes various program components, including triage and case assignment procedures, referrals, liaison activities, and staff scheduling to ensure adequate coverage at all times. Maintains records and statistics in accordance with program requirements.

Participates in follow up programs, surveys, and research activities as required by the program. Participates in interdisciplinary team meetings, individual and group supervisory meetings and required trainings. Assists or facilitates onboarding of new hires.

Works with Program Manager to review compliance of documentation and readiness for billing. Participates in special projects and performs other duties as assigned. Qualifications Valid New York State driver’s license may be required based on operational needs. required Master's Degree in Social work, Mental Health Counseling or in a related field required Minimum of two years of experience providing mental health services required Minimum one year of supervisory experience preferred
